Meals on Wheels provides hot, nutritious meals, a daily safety check and friendly interaction to homebound seniors in Coos County who are unable to shop or cook for themselves, helping them to maintain a healthy lifestyle and remain living independently in their homes for as long as possible.

Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD) provides education, advocacy, and support. It provides information and support for: adults, children, families, parents, professionals, and teens. Information and one on one help is provided through a toll free information line. It is an information clearinghouse. It has webinars, online and in person peer support groups, parent support and education, tips for living with ADHD, and more. Boulder Point Veteran Housing is located in Plymouth, New Hampshire. It offers 25 one-bedroom apartments dedicated to veterans experiencing homelessness, through partnership with both the White River Junction Veteran Affair Medical Center and the US Department of Housing and Urban Development's Veteran Supportive Housing vouchers (HUD-VASH Program). The facility also offers five two-bedroom affordable (not subsidized) two-bedroom apartments. Check for current openings or contact us for more information and to see if you qualify. Location & Amenities: A 27,000 square foot, 2-story, handicapped accessible/adaptable building, Boulder Point Veteran Housing is located on a picturesque 11-acre property. It features plenty of parking and outdoor space, as well as a large community room, a library/ game room, in-unit washer and dryers, and several private offices for residents to receive case management and supportive services. The program provides mentoring, case management, and supportive services for residents who choose to participate. All participants receive on-site access to coaching in eviction prevention, increasing household income and assets, increasing health and well-being, general life skills, and improving the educational success of children and youth.

Through our Helping Ensure Appropriate Response to Trauma (H.E.A.R.T) program, CACs are partnering with school districts to address trauma related needs of school aged children. The H.E.A.R.T program is a collaboration between school districts and the Greater Lakes Child Advocacy Center in order (with parental permission) to inform appropriate school personnel when a child has been seen at the CAC, so that school staff can respond to the child’s needs in an appropriate, trauma informed way.

You maintain full control over your plan, know­ing that your IL Services Coordinator is there to answer questions and make recommendations when you need support. Kinship Navigator Services provides services to people caring for a child with a kinship arrangement, which includes adoption, DCYF placements, foster care, legal guardianships, and informal guardianship. Kinship Navigator help connect you to:

• Services and support that meet the needs of both children and caregiver
• Emotional support
• Education and guidance
• Information and referral
• Follow-up services to promote independence and enhance the well-being of the family

The Support Services Fund assists applicants with essential daily living expenses as they go through diagnosis, treatment and recovery. Typical requests include rent, mortgage, utilities, car repair, transportation, childcare, groceries, and minor medical expenses, including lymphedema supplies. We are not able to consider major medical expenses.

In collaboration with local hospitals and health clinics, services offered includes, blood pressure screenings, foot screening and care, flu vaccines and other health and wellness checks.

We want to encourage healthy hygiene, minimize infections and increase quality of life while providing outreach material, information, and screenings to ensure that needs are met, and safety is maximized. The Youth Law Project (YLP) works with children and teenagers who are facing delinquency or CHINS petitions, as well as those who are at-risk of having such petitions filed against them. The YLP seeks to get these young people the educational, health, mental health, and other services necessary to stay in their homes and communities and out of the juvenile justice system. The type of work the YLP does includes representation at school disciplinary hearings and special education meetings and proceedings. The YLP does not provide representation in delinquency or CHINS cases, but works closely with the public defenders and court-appointed counsel who do to determine how best to meet the youth’s needs. The Youth Law Project handles the following types of cases: Special education advocacy and representation at educational legal proceedings for children in the juvenile justice system and at risk of court involvement. Representation at long term suspension and expulsion hearings. Representation at other legal proceedings that may affect a youth's right to attend school.

The Board of Nursing provides licences and certifications to people in the nursing field. They also protect the people of the State from the unauthorized, unqualified, and improper application of services by individuals in the practice of nursing. The Board regulates:

NH Housing received a special allocation of vouchers for homeless veterans. These vouchers come through a special screening process which is handled by the Veterans Administration. The program is governed by several special regulations with regard to initial criminal screening and case management.

Quality of Life Grant program for people with ALS in the Northern New England Chapter. This is a reimbursement grant program and you may be reimbursed up to $600 per year for costs associated with ALS. Requests are subject to the availability of funds at the time of submission.

Young adults who were in the legal custody of NH DHHS upon attaining the age of 18 have the option of voluntarily requesting to remain in DCYF foster care until the age of 21. This includes young adults that previously declined this option at the time of their 18th birthday or started the program and discontinued participation.

The Homelessness Prevention and Rapid Re-Housing Program will provide financial assistance and services to prevent individuals and families from becoming homeless and help those who are experiencing homelessness to be quickly re-housed and stabilized. The funds under this program are intended to target individuals and families who would be homeless but for this assistance. The funds will provide for a variety of assistance, including: short-term or medium-term rental assistance and housing relocation and stabilization services, including such activities as mediation, credit counseling, security or utility deposits, utility payments, moving cost assistance, and case management.
