The Community Integration Program (CIP) includes comprehensive wrap-around services available 24 hours per day, 7 days per week, that vary in support and intensity based on the participant’s needs. Services are designed to support individuals in building and sustaining skills for successful independent living in the community. Our philosophy is based on principles of the Assertive Community Treatment (ACT) model, an evidence-based practice that is designed specifically for individuals with serious mental illness with or without co-occurring substance use disorders. Some of these principles include embracing a holistic approach to treatment, providing integrated services by one team, promoting continuity of care, and building upon natural supports to successfully maintain community living. While success and recovery are defined by each individual and family we work with, WestBridge helps individuals gain the skills needed to live a healthy, recovery-oriented and satisfying life.

In our Transitional Housing for Veterans, veterans and their families live in private apartments in one of our three apartment complexes located in Nashua and Manchester, NH. While living in Transitional Housing, veterans pursue employment, further their education, or expand their skills by volunteering within the community. They work with closely with both a case manager and, as needed, an employment specialist to permanently escape homelessness. Through our integrated services, as well as community partners, they also access health care, transportation, and meaningful income. This program is sponsored through a US Department of Veteran Affairs grant. By the end of their stay, veterans have the income and healthcare they need to live independently, with pride and dignity. Locations: Harbor Homes currently operates three transitional housing facilities throughout NH. --Buckingham Place is a 20-unit one and two bedroom apartment complex at 46 Spring Street in Nashua. The facility provides handicapped accessible apartments as well as common areas and a large community room. Named “Buckingham Place” after Byron Buckingham, a local veteran advocate, the facility was the first of its kind in the nation to house male and female veterans as well as their families. --Dalianis House is a 40-unit apartment complex located at 59 Factory Street in Nashua. In 2010, Harbor Care spearheaded a massive renovation of the property (once a furniture outlet), and now serves male veterans in one bedroom and single room occupancy apartments. --BAE Systems Independence Hall is a 26-unit one and two bedroom apartment complex for male and female veterans and their families, located at 335 Somerville Street in Manchester. Six of the apartments house veteran households permanently in supportive housing.

Supported Employment is an evidence-based practice designed to help individuals with severe and persistent mental illness to find and keep competitive jobs. Because work can be an essential step on the path to well- being and recovery, our Vocational Specialists help clients find work and stay employed. They help the client assess their skills and vocational interests, search for a job, prepare for the interview and provide ongoing support and guidance to help the client remain employed. In some instances, family members or a significant other may be part of the support team.
