Direct care services to New Hampshire community members living with HIV/AIDS. --Medical Case Management, Housing, & More: The heart of our program, our experienced case managers link clients with health care, housing, all Harbor Care services, as well as transportation services and healthy food through our pantry. We may also extend emergency rent and utility payment assistance to those who qualify to prevent homelessness. --Peer Support Services: We host groups and provide one-on-one support for individuals living with HIV/ AIDS.

The Community Integration Program (CIP) includes comprehensive wrap-around services available 24 hours per day, 7 days per week, that vary in support and intensity based on the participant’s needs. Services are designed to support individuals in building and sustaining skills for successful independent living in the community. Our philosophy is based on principles of the Assertive Community Treatment (ACT) model, an evidence-based practice that is designed specifically for individuals with serious mental illness with or without co-occurring substance use disorders. Some of these principles include embracing a holistic approach to treatment, providing integrated services by one team, promoting continuity of care, and building upon natural supports to successfully maintain community living. While success and recovery are defined by each individual and family we work with, WestBridge helps individuals gain the skills needed to live a healthy, recovery-oriented and satisfying life.

SE services include the development of community-based employment, supplemental job training, and long-term support. Job training is provided in addition to the training the employer naturally provides. In addition, our role is to assist with the individual’s independence level through the development of natural supports.

The Housing Program assists local communities within Belknap and Merrimack Counties in the development of affordable housing for seniors and adults with disabilities. These are independent living facilities and are rent subsidized. Tenants are required to pay 30% of their adjusted income for rent. Senior housing properties are in the following towns: Alton, Belmont, Epsom, Warner, Newbury and Pembroke

Energy and Utility Justice Project, NHLA advocates for access to affordable basic utility services for low-income consumers. NHLA represents clients with issues related to utility disconnections and arrears as well as with issues accessing assistance programs such as the Electric Assistance Program, the Weatherization Assistance Program, the Fuel Assistance Program, and energy efficiency programs.

Provide support, guidance, helpful information and positive role modeling; help to develop parenting skills such as behavior management, proper nutrition, healthy lifestyles, and more. Provide short term supervised visitation. Help to connect with resources for food, housing, transportation, utilities, education, job opportunities, etc.

Provides cell phones for seniors to call 911 in an emergency.

Riverbend offers a continuum of residential options for people with psychiatric disabilities, including supervised apartments, short-term crisis and transitional housing, group homes, specialized services for elders and long-term residential care.

Lunchtime at Horsemeadow provides a chance for community members to get together for a visit with friends as they enjoy a hot nutritious meal. Menus for the month are in the newsletters or through the local newspaper. Volunteers bring a meal each weekday right to the door of homebound elderly neighbors.

The Seven Challenges is a comprehensive counseling program for youth and young adults that incorporates work on alcohol and other drug problems. It is designed to motivate youth and young adults to evaluate their lives, consider changes they may wish to make, and then success in implementing the desired changes. It supports them in taking power over their own lives.

Eligible youth will receive a Housing Choice Voucher for a period of 3 years. Services to assist the youth in applying, searching for, moving in to, and maintaining housing will be provided either by DCYF or a community agency. Assistance may include financial assistance with rental and utility deposits.

In our Transitional Housing for Veterans, veterans and their families live in private apartments in one of our three apartment complexes located in Nashua and Manchester, NH. While living in Transitional Housing, veterans pursue employment, further their education, or expand their skills by volunteering within the community. They work with closely with both a case manager and, as needed, an employment specialist to permanently escape homelessness. Through our integrated services, as well as community partners, they also access health care, transportation, and meaningful income. This program is sponsored through a US Department of Veteran Affairs grant. By the end of their stay, veterans have the income and healthcare they need to live independently, with pride and dignity. Locations: Harbor Homes currently operates three transitional housing facilities throughout NH. --Buckingham Place is a 20-unit one and two bedroom apartment complex at 46 Spring Street in Nashua. The facility provides handicapped accessible apartments as well as common areas and a large community room. Named “Buckingham Place” after Byron Buckingham, a local veteran advocate, the facility was the first of its kind in the nation to house male and female veterans as well as their families. --Dalianis House is a 40-unit apartment complex located at 59 Factory Street in Nashua. In 2010, Harbor Care spearheaded a massive renovation of the property (once a furniture outlet), and now serves male veterans in one bedroom and single room occupancy apartments. --BAE Systems Independence Hall is a 26-unit one and two bedroom apartment complex for male and female veterans and their families, located at 335 Somerville Street in Manchester. Six of the apartments house veteran households permanently in supportive housing.

Kinship Navigator Services assist caregivers of children in various kinship arrangements, offering tailored support, emotional guidance, educational resources, and referrals to promote family well-being and independence.

Supported Employment is an evidence-based practice designed to help individuals with severe and persistent mental illness to find and keep competitive jobs. Because work can be an essential step on the path to well- being and recovery, our Vocational Specialists help clients find work and stay employed. They help the client assess their skills and vocational interests, search for a job, prepare for the interview and provide ongoing support and guidance to help the client remain employed. In some instances, family members or a significant other may be part of the support team.

The NH Works for Recovery program provides comprehensive and individualized re-employment services to individuals who have been directly or indirectly affected by the opioid crisis. Eligible participants will have an opportunity to receive job training and support services to help them successfully compete for many of the in-demand occupations located throughout New Hampshire.

Support group for those affected by heart disease or for those who have had heart surgery and their family and friends.

Substance use disorder counseling with or without a co-occurring mental health diagnosis.

A 5-day immersive program for individuals aged 55+ affected by severe vision impairment. These Retreats focus on communication skills, orientation & mobility, accessible technology, low vision skills and devices, independent living skills, adjustment to vision loss, social interaction, and peer support.

Quality of Life Grant program for people with ALS in the Northern New England Chapter. This is a reimbursement grant program and you may be reimbursed up to $600 per year for costs associated with ALS. Requests are subject to the availability of funds at the time of submission.
