For CFI/ABD/DD services, request Brain Injury Association of NH during your intake.
Fees
Waivered Case Management services are covered under Medicaid.
Private Pay case management is billed directly to the individual.

Case management services assist individuals in identifying needs, identifying services providers, securing appropriate providers and ensuring services are provided timely. Case Management will also assist with paperwork regarding benefits, housing, etc. Case Management assists individuals in advocating for themselves.
