We advocate and oversee supports and services to ensure continuity of care. One Sky offers a variety of support, day and residential services for children and adults with developmental disabilities and traumatic/acquired brain disorders. Each service is individually tailored in conjunction with a person's circumstances, needs and desires. Services includes: Service Coordination; In-Home Support Services; Environmental Modifications; One Sky Family Services Transition Coordinator; and Respite Care

Provides services to children and teens with Autism and their families and caregivers. Service examples are: home based Applied Behavior Analysis, diagnosis, assessment, therapy, camps, school support, in home respite care, parent and caregiver trainings, and more

Adult Service Coordination team at MDS assists adults with developmental and related disabilities as they identify personal goals, and then secures the supports and resources required to achieve them. Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process

Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

Life planning assistance for adults with developmental disabilities, acquired brain injuries and their family members, as well as veterans.

Provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay.

The Autism Center helps children, youth, young adults, and their families. The Center uses Applied Behavior Analysis (ABA). Parents receive training and strategies they can use in their natural family routines. The Center helps youth and young adults with autism and their families plan for adulthood.

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

We partner with families to meet the developmental needs of the child for children birth to age 3. For those children found eligible, we offer services that include parent coaching, physical, occupational, speech and developmental therapies.

A safe and welcoming place where adults with special needs, their families and caregivers, and caring members of the Upper Valley community can come together and take part in fun and educational activities. Programs for adults with disabilities include: arts, cooking, crafts, employment skills, gardening, health and fitness, life skills, music, science, and outings. Free support group for families and caregivers.
