Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process

A safe and welcoming place where adults with special needs, their families and caregivers, and caring members of the Upper Valley community can come together and take part in fun and educational activities. Programs for adults with disabilities include: arts, cooking, crafts, employment skills, gardening, health and fitness, life skills, music, science, and outings. Free support group for families and caregivers.

The Moore Center serves two main groups of people, those with Intellectual & Developmental Disabilities and Acquired Brain Disorders for adults and children. - Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. - Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. - Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services; we help those who are 18 or older to identify and apply for appropriate state and federal benefits; and we work with young adults who want to live in the community and need minimal supports to maintain their independence..

Service Coordination: We offer case management support to individuals with intellectual disabilities and brain acquired injuries. We advocate and oversee supports and services to ensure continuity of care. Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

Life planning assistance for adults with developmental disabilities, acquired brain injuries and their family members, as well as veterans.

Provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay.
