The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

Our staff helps NH and VT families find resources and services to meet basic needs, make connections, and get support. We offer a wide range of educational classes and groups for free, and all are welcome. For NH families, we provide Healthcare Coordination for families raising children with chronic healthcare conditions, and Community-Based Voluntary Services to preserve families at risk.
