DEVELOPMENTAL DISABILITIES AND BRAIN INJURIES | MONADNOCK DEVELOPMENTAL SERVICES
Adult Service Coordination team at MDS assists adults with developmental and related disabilities as they identify personal goals, and then secures the supports and resources required to achieve them. Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process
