Description

Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.
