PathWays provides supports for children with disabilities, adults with developmental disabilities or brain injuries in everyday life to maintain their independence and well-being is what we do here at Pathways. Services includes: Service Coordination, Community Support Services, Enhanced Family Care, Community Participation Services and Residential Supports.

The Moore Center serves two main groups of people, those with Intellectual & Developmental Disabilities and Acquired Brain Disorders for adults and children. - Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. - Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. - Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services; we help those who are 18 or older to identify and apply for appropriate state and federal benefits; and we work with young adults who want to live in the community and need minimal supports to maintain their independence..

Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process

Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

We offer support at education meetings, guide you to community resources, and help with the transition into adult life. Children's In Home Supports - An intensive home based program helping children with complex medical and behavioral needs. Services that are offered are personal care, service coordination, consultative services (including behavioral), respite care, and environmental modifications.
