The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. One Sky offers a variety of support, day and residential services for children and adults with developmental disabilities and traumatic/acquired brain disorders. Each service is individually tailored in conjunction with a person's circumstances, needs and desires. Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

We work with individuals age 3-21 that experience a developmental disability and live with a family member. Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.
