Eligibility
Individuals who are eligible include those who:
- Participate in an Independent Living Program
- Are Medicaid eligible (State Plan)
- Have a physical disability *Require a wheelchair for mobility
- Require at least 2 hours of personal care services daily
- Are 18 years of age or older
- Are their own legal guardian
- Are able to self-direct their care

Office hours: Mon. - Fri., 8 am - 4 pm,

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.
