Kinship Care enable loved ones to enjoy life in a family home instead of a nursing home. The program ensures care is provided in a family setting for those who can no longer live safely and independently in their own home. Kinship Care program enables families to be compensated for caring for a loved one at home, in lieu of having them placed in a nursing home.

Some of the supports and services available (ages 3 to 20) include: Autism Supports; In Home Supports; Parent-to-Parent Program; High School Transition; and Futures Planning. Services include: Information and referral to services and resources; Respite Services; Respitality Programs; Recreation and Summer Camps; Funding assistance and variety of assistive technology products.

Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

In Home Meal Preparation
Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.
