Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Services for adults and children with developmental disabilities and/or acquired brain injuries include case management, day programs, employment programs, family support and residential services. Some of the supports and services available (ages 3 to 20) include: Autism Supports; In Home Supports; Parent-to-Parent Program; High School Transition; and Futures Planning. Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

Community Mental Health Center for Cheshire County - Region V. Personal counseling, care management and other MFS services are available at all of our locations throughout the Monadnock region. Some clinical services are available in Keene and Peterborough and others are provided in the community.

Kinship Care enable loved ones to enjoy life in a family home instead of a nursing home. The program ensures care is provided in a family setting for those who can no longer live safely and independently in their own home. Kinship Care program enables families to be compensated for caring for a loved one at home, in lieu of having them placed in a nursing home.
