Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

Personal Care: We offer clients who require ongoing or long-term support and assistance with personal care with professional service on a private fee basis. These services include: Assist with personal care, Incontinence care, Meal preparation Homemakers: Assists in household management activities, such as: light housekeeping, cleaning, laundry, grocery shopping, errands, preparing and serving of simple meals. Senior Companions: Provide friendly visiting, reading, writing, errands and companionship.

Services for adults and children with developmental disabilities and/or acquired brain injuries include case management, day programs, employment programs, family support and residential services. Some of the supports and services available (ages 3 to 20) include: Autism Supports; In Home Supports; Parent-to-Parent Program; High School Transition; and Futures Planning. Services include: Information and referral to services and resources; Respite Services; Respitality Programs; Recreation and Summer Camps; Funding assistance and variety of assistive technology products.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. Additional Services may include: Service Coordination, Respite, High School Transition Supports, In Home Support Services, Personal Care, Mobility and Physical Functioning, Consultation and Education, Functional Supports and Services.

Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.
