Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

Services for adults and children with developmental disabilities and/or acquired brain injuries include case management, day programs, employment programs, family support and residential services. Some of the supports and services available (ages 3 to 20) include: Autism Supports; In Home Supports; Parent-to-Parent Program; High School Transition; and Futures Planning. Services include: Information and referral to services and resources; Respite Services; Respitality Programs; Recreation and Summer Camps; Funding assistance and variety of assistive technology products.

Early Intervention includes speech, occupational, and physical therapy; counseling; and education interventions for children up to age 3 with a confirmed or suspected developmental delay or disability.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. Lamprey Health Care is a non-profit health care clinic. The clinic provides a wide range of primary health care services to people of all ages. Other services provided are counseling, HIV testing, chronic disease management, physicals, wellness programs, diabetes screening and education, and more. Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

We work with individuals age 3-21 that experience a developmental disability and live with a family member. We offer support at education meetings, guide you to community resources, and help with the transition into adult life. Children's In Home Supports - An intensive home based program helping children with complex medical and behavioral needs. Services that are offered are personal care, service coordination, consultative services (including behavioral), respite care, and environmental modifications.

Special Medical Services are part of the State of New Hampshire's Title V Programs for Children with Special Health Care Needs. Current programs include: Child Development Clinic; Community Based Care Coordination; Nutrition, Feeding and Swallowing; and Neuromotor Clinic. Case management is provided to children and their families who are enrolled.

Outpatient Specialty Care Services includes: - Center for Medical Genetics: prenatal and adult genetic testing and counseling - Neurology Services specialize in the prevention, diagnosis and management of an array of neurological disorders, including headaches, autism, ADHD, neuropathy, sleep disorders, small-fiber polyneuropathy, stroke and more. - Endocrinology & Diabetes Consultants: endocrinology, diabetes, metabolism and obesity medicine. - Arthritis & Osteoporosis Center: full-service rheumatology practice caring for adults with arthritis, osteoporosis, musculoskeletal pain disorders and systemic autoimmune diseases. - Cardiology Services & Vascular Surgery: provide h diagnosis, treatments and preventive care for heart patients dealing with high blood pressure, valve disease, heart failure, and other forms of heart disease.

The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.
