The Portsmouth Housing Authority (PHA) provides subsidized housing for families, elderly and people with disabilities. The housing is for low and extremely low income individuals and families. ***Currently there are waiting lists for all housing and housing programs. The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

We provide nutritious meals delivered by trained drivers, daily safety checks, resource information, and peace of mind for the participants and their loved ones. Meals are available short or long term, depending on the need. Meals are available five days a week, depending on the need. Shelf stable meals are provided for holidays and weather emergencies

Freshly prepared healthy meal is delivered each weekday by our friendly and trusted deliverer. Includes safety focus checks, where trained staff is familiar with the needs and the living environment of each participant.

Provides transportation to medical appointments. This is not for public use. You must be a registered client will the program.

Hot noon-time meals are provided to the elderly and/or disabled, Monday through Friday, delivered to their homes by volunteer drivers. Meals are made fresh daily and are prepared under the direction of the Food & Nutrition Department of Huggins Hospital.

Our transportation services include: • Wheelchair Van Transportation • Local Transit Travel Training • Travel Reimbursement • Non-emergency Medical Transportation • ADA (Americans with Disabilities Act) Para-transit Application Assistance 1. Granite State Independent Living (GSIL) offers various transportation options, programs, and services for adults with disabilities. Transportation is offered for: medical appointments, social outings such as movies, shopping, family gatherings, events, etc. 2. Requests for transportation should be made at least 5 days in advance. 3. GSIL has a Transportation Reimbursement Program (TRP) for qualified consumers. This is for consumers who do have access to public transportation in their area or they have someone who is willing to give them a ride.

Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

For riders who have a disability that prevents them from making some or all of their trips on fixed-route buses, CAT offers a shared-ride, origin-to-destination service called ADA paratransit service. ADA paratransit service operates up to ¾ of a mile on either side of CAT’s fixed-route bus service during the same days and hours as the fixed-route bus service.

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

The Salem Housing Authority operates public housing for older adults (62+) and adults with disabilities head or co-head of household. To be added to our waiting list, the individual/s must fill out an application to determine eligibility and if found to meet criteria, are added to the wait list. **Emergency housing services are not available.

Provided through a variety of models that meet each person's unique needs, options include a home staffed by Easterseals employees who provide support/assistance; placement in the home of a family or couple responsible for providing support/assistance; or identification and supervision of a roommate to live with the individual to provide support/assistance.
