Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

Free minor home repairs for under-resourced families, the elderly and families with physically challenged individuals, performed by Jr. & Sr. High School students. Paint, drywall, small & easy flooring and repairs to: decks, ramps siding. Small low pitch, low to the ground roofing are all within their skill set. Students cannot perform electrical, plumbing nor structural tasks.

Meals on Wheels regularly provides the healthy foods you want, allowing you to live independently in your own home. Whether you want the convenience of a healthy and ready-to-eat meal, or are unable to prepare nutritious meals for yourself, you can receive meals from Meals on Wheels

Individualized, community-based residential services provided to children and adults with disabilities. Provided through a variety of models that meet each person's unique needs, options include a home staffed by Easterseals employees who provide support/assistance; placement in the home of a family or couple responsible for providing support/assistance; or identification and supervision of a roommate to live with the individual to provide support/assistance.

Meals on Wheels are provided by volunteers each weekday to homebound seniors. No meals are delivered on major holidays or announced "snow days," so volunteers bring an extra frozen meal to use when fresh meals are not delivered. Anyone with health concerns that require a special diet should let the program know, so their needs can be accommodated. All meals are prepared with no added salt.

The Claremont Housing Authority (CHA) has one public housing location, which is the 96-unit Marion L. Phillips Apartments located at 243 Broad St., Claremont, NH 03743. CHA also issues 140 vouchers for the Section 8/Housing Choice Voucher program.

SCS Transportation provides bus routes in the towns of Charlestown and Newport as well as the City of Claremont to the public.

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

We provide nutritious meals delivered by trained drivers, daily safety checks, resource information, and peace of mind for the participants and their loved ones. Meals are available short or long term, depending on the need. Meals are available five days a week, depending on the need. Shelf stable meals are provided for holidays and weather emergencies

Provides transportation to medical appointments. This is not for public use. You must be a registered client will the program.

Hot noon-time meals are provided to the elderly and/or disabled, Monday through Friday, delivered to their homes by volunteer drivers. Meals are made fresh daily and are prepared under the direction of the Food & Nutrition Department of Huggins Hospital.

Granite State Independent Living offers a variety of services geared to helping people with disabilities access their community-based transportation options throughout the State of New Hampshire. Our transportation services include: • Wheelchair Van Transportation • Local Transit Travel Training • Travel Reimbursement • Non-emergency Medical Transportation • ADA (Americans with Disabilities Act) Para-transit Application Assistance 1. Granite State Independent Living (GSIL) offers various transportation options, programs, and services for adults with disabilities. Transportation is offered for: medical appointments, social outings such as movies, shopping, family gatherings, events, etc. 2. Requests for transportation should be made at least 5 days in advance. 3. GSIL has a Transportation Reimbursement Program (TRP) for qualified consumers. This is for consumers who do have access to public transportation in their area or they have someone who is willing to give them a ride.

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

Stepsaver is a transportation program offered by the Manchester Transit Authority as required under the Americans with Disabilities Act (ADA) of 1990. This Act requires the MTA to provide a "complementary paratransit program" to individuals who are prevented from using the fixed route bus service. Stepsaver is a curb-to-curb transportation service that provides assistance to individuals who are prevented from using the regular fixed route bus service. Some customers may require the assistance of a Personal Care Attendant (PCA) to ride the vehicle. In order to be registered as a PCA, the eligible customer must have a physician complete a PCA certification form indicating that the customer must have a PCA. PCAs ride for free with the eligible customer. Customers may also bring along paying guests when space is available. No more than three (3) guests may ride at any time.

Provides fixed route and curb-to-curb demand-response transportation serving the NH towns of Chester, Derry, Hampstead, Londonderry and Salem. All buses are wheelchair accessible.

Easter Seals' statewide Caring Companions Program provides non-medical home care services for: older adults, people with a chronic illness, and people with disabilities.

Transportation is available to medical or other appointments, shopping, errands, and the senior center. In addition to transportation provided by bus, volunteer drivers also are available to help older adults.Some senior centers are able to offer long-distance medical transportation to those who need to reach a specialist or a major medical center not located in their local community. COAST provides advance reservation-based ADA paratransit service. A person may qualify for this service if the person has a disability that prevents the person from independently getting to a bus stop, riding the bus, navigating the route or identifying the bus. -This is a reservation based services within 3/4 mile of the fixed route system. -Eligible persons must call at least 1 business day ahead to schedule a ride. -Passengers must pay a fare each time. -Not everyone who has a disability will qualify.

A safe and welcoming place where adults with special needs, their families and caregivers, and caring members of the Upper Valley community can come together and take part in fun and educational activities. Programs for adults with disabilities include: arts, cooking, crafts, employment skills, gardening, health and fitness, life skills, music, science, and outings. Free support group for families and caregivers.
