Volunteer driver service for medical related appointments for people age 55 + and people with disabilities who are unable to drive. Accessible transportation is available.

We provide nutritious meals delivered by trained drivers, daily safety checks, resource information, and peace of mind for the participants and their loved ones. Meals are available short or long term, depending on the need. Meals are available five days a week, depending on the need. Shelf stable meals are provided for holidays and weather emergencies

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

The TripLink Common Application allows users to apply for many of the region's nonprofit transportation options all at the same time. Based on your age, residency, and disability status, the Common Application will prompt you to apply for all the services for which you might be eligible. This process can be completed completely online, or you may submit some documents by mail or fax if that is easier for you. Meals on Wheels are provided by volunteers each weekday to homebound seniors. No meals are delivered on major holidays or announced "snow days," so volunteers bring an extra frozen meal to use when fresh meals are not delivered. Anyone with health concerns that require a special diet should let the program know, so their needs can be accommodated. All meals are prepared with no added salt.

SCS Transportation provides bus routes in the towns of Charlestown and Newport as well as the City of Claremont to the public.

Provided through a variety of models that meet each person's unique needs, options include a home staffed by Easterseals employees who provide support/assistance; placement in the home of a family or couple responsible for providing support/assistance; or identification and supervision of a roommate to live with the individual to provide support/assistance.

Individuals that qualify for the Choices for Independence (CFI) waiver may choose Community Crossroads for their Case Management. Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. Typical services may include but are certainly not limited to: Personal Care; Home Health Aides; Homemakers; Nursing; Meals on Wheels; Adult Medical Day Services; Personal Emergency Response Systems.

Easter Seals' statewide Caring Companions Program provides non-medical home care services for: older adults, people with a chronic illness, and people with disabilities.

Transportation is available to medical or other appointments, shopping, errands, and the senior center. In addition to transportation provided by bus, volunteer drivers also are available to help older adults.Some senior centers are able to offer long-distance medical transportation to those who need to reach a specialist or a major medical center not located in their local community. If for any reason you are unable to get out or prepare a meal yourself, our Meals On Wheels volunteers will be happy to bring meals right to you for as long as you require them. We will also provide caregivers with a meal if they wish. Meals are delivered in a special container. You may eat it when it arrives or refrigerate or freeze it for later. As long as funding permits, we send breakfast items and frozen dinners on Fridays

Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

Provides fixed route and curb-to-curb demand-response transportation serving the NH towns of Chester, Derry, Hampstead, Londonderry and Salem. All buses are wheelchair accessible.

Rides are provided for medical and personal appointments: doctor/dentist appointments, lab tests, pharmacy pick-ups, grocery and other shopping, errands, hair appointments, etc. We will accommodate extended-area trips (i.e., Boston, Manchester, Lebanon, Portland, etc.) for medical appointments only. **Rides are provided by volunteers in their personal vehicles. We are unable to accommodate wheelchairs.

A safe and welcoming place where adults with special needs, their families and caregivers, and caring members of the Upper Valley community can come together and take part in fun and educational activities. Programs for adults with disabilities include: arts, cooking, crafts, employment skills, gardening, health and fitness, life skills, music, science, and outings. This mobile food pantry program provides and delivers free groceries to eligible neighbors in need. From the taking and packing of individualized food orders to the personally delivered groceries, this program is totally volunteer driven.

Provides: - Animal related advice and counseling -Transportation to pet related appointments - Pet Partner Handler training and evaluations for volunteers & pets wishing to visit facilities and schools - Financial assistance for unexpected veterinary care in the form of no-interest loans (Strafford and Seacoast Rockingham counties) - Rx medication assistance for chronic conditions in pets

Meals on Wheels provides hot, nutritious meals, a daily safety check and friendly interaction to homebound seniors in Coos County who are unable to shop or cook for themselves, helping them to maintain a healthy lifestyle and remain living independently in their homes for as long as possible.

COAST provides advance reservation-based ADA paratransit service. A person may qualify for this service if the person has a disability that prevents the person from independently getting to a bus stop, riding the bus, navigating the route or identifying the bus. -This is a reservation based services within 3/4 mile of the fixed route system. -Eligible persons must call at least 1 business day ahead to schedule a ride. -Passengers must pay a fare each time. -Not everyone who has a disability will qualify.
