Granite State Independent Living offers a variety of services geared to helping people with disabilities access their community-based transportation options throughout the State of New Hampshire. Our transportation services include: • Wheelchair Van Transportation • Local Transit Travel Training • Travel Reimbursement • Non-emergency Medical Transportation • ADA (Americans with Disabilities Act) Para-transit Application Assistance 1. Granite State Independent Living (GSIL) offers various transportation options, programs, and services for adults with disabilities. Transportation is offered for: medical appointments, social outings such as movies, shopping, family gatherings, events, etc. 2. Requests for transportation should be made at least 5 days in advance. 3. GSIL has a Transportation Reimbursement Program (TRP) for qualified consumers. This is for consumers who do have access to public transportation in their area or they have someone who is willing to give them a ride.

Call to speak to someone in Information & Referral department for eligibility *** Fee for Service that are NOT Medicaid, call Transportation Department: (603) 228-9680 *** All non emergency medical Medicaid appointments must go through Medicaid Provider the telephone number is on the back of your Medicaid card. Granite State Independent Living cannot set up any Medicaid medical appointment trips directly.
