Direct care services to New Hampshire community members living with HIV/AIDS. --Medical Case Management, Housing, & More: The heart of our program, our experienced case managers link clients with health care, housing, all Harbor Care services, as well as transportation services and healthy food through our pantry. We may also extend emergency rent and utility payment assistance to those who qualify to prevent homelessness. --Peer Support Services: We host groups and provide one-on-one support for individuals living with HIV/ AIDS.

Harbor Care is uniquely designed to support your recovery journey-with or without insurance. No one is turned away, regardless of ability to pay. A full range of quality residential and outpatient services are offered so that we can accompany clients on their journeys from the first time they consider treatment to long-term recovery. We offer: outpatient treatment, including our High Intensity Outpatient Program (PHP and IOP); treatment for pregnant and parenting women and their children at the Cynthia Day Family Center; Medication Assisted Treatment (MAT) for opioid and/or alcohol dependence; long-term recovery support counseling and groups statewide with our Peer Recovery Support Services; and services for the Rockingham County Drug Court. We're dedicated to partnering with schools to ensure the health, safety, and well-being of its staff, student body and their families. We work on-site and through telehealth, side-by-side with school social workers, nurses, teachers and administrators to being health care and supportive services not just to the students who need it most, but also students' families and school faculty. We're with our community all of the way! We help youth and their loved ones access specialty care providers and social services, often working with our school nurse partners. We also connect families with systems and ensure long-term supports are in place for students with behavioral health issues. With specialized treatment that may include child care and transportation for parents in need of behavioral health care, we take a holistic, whole-family approach to ensuring health and wellness.

Housing Opportunities for Persons with AIDS (HOPWA), is a federal grant made available by The United States Department of Housing and Urban Development (HUD) which is focused on addressing the housing needs of people living with HIV/AIDS.

Services includes: Case Management; Food Pantry; Transportation to and from Medical Appointments; Assistance in Securing and Maintaining Safe, Affordable Housing; Emergency Rent and Utility Payment Assistance; Linkages to Mental Health Counseling; Linkages to Substance Use Disorder Counseling; Linkages to Partner Notification of HIV/AIDS status and Testing; Linkages to HIV Medical Care Providers and other AIDS Service Organizations

The CARE program offers eligible NH residents living with HIV/AIDS access to healthcare and support services. Clients enroll in the NH CARE Program through MVAP.

Offers medical case management services to individuals living in Grafton, Sullivan and Coos counties in New Hampshire and Windsor and Orange counties in Vermont. Services offered through Case Management include but are not limited to: Financial Assistance; Food Assistance; Housing Assistance; Medical Assistance; Transportation and Referrals
