The HIV/HCV Resource Center offers medical case management services to individuals living in Grafton, Sullivan and Coos counties in New Hampshire and Windsor and Orange counties in Vermont. Services offered through Case Management include but are not limited to: Financial Assistance; Food Assistance; Housing Assistance; Medical Assistance; Transportation and Referrals.
