Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Adult Service Coordination team at MDS assists adults with developmental and related disabilities as they identify personal goals, and then secures the supports and resources required to achieve them. Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

Easter Seals' statewide Caring Companions Program provides non-medical home care services for: older adults, people with a chronic illness, and people with disabilities.

Case management services offered Homemakers and Companions provide light housekeeping, meal preparation and errand service. Services for new moms needing homemaking services, also can be provided to anyone for a fee.

Granite VNA offers in home assistance to provide the family of some relief. A program is tailored to meet the need in the areas of homemaking and personal care while providing companionship for the client.

The Moore Center serves two main groups of people, those with Intellectual & Developmental Disabilities and Acquired Brain Disorders for adults and children. - Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. - Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. - Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services; we help those who are 18 or older to identify and apply for appropriate state and federal benefits; and we work with young adults who want to live in the community and need minimal supports to maintain their independence..

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

PathWays provides supports for children with disabilities, adults with developmental disabilities or brain injuries in everyday life to maintain their independence and well-being is what we do here at Pathways. Services includes: Service Coordination, Community Support Services, Enhanced Family Care, Community Participation Services and Residential Supports.

One Sky offers a variety of support, day and residential services for children and adults with developmental disabilities and traumatic/acquired brain disorders. Each service is individually tailored in conjunction with a person's circumstances, needs and desires. Services includes: Service Coordination; In-Home Support Services; Environmental Modifications; One Sky Family Services Transition Coordinator; and Respite Care
