Individualized, community-based residential services provided to children and adults with disabilities. Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

We work with individuals age 3-21 that experience a developmental disability and live with a family member. We offer support at education meetings, guide you to community resources, and help with the transition into adult life. Children's In Home Supports - An intensive home based program helping children with complex medical and behavioral needs. Services that are offered are personal care, service coordination, consultative services (including behavioral), respite care, and environmental modifications.

One Sky offers a variety of support, day and residential services for children and adults with developmental disabilities and traumatic/acquired brain disorders. Each service is individually tailored in conjunction with a person's circumstances, needs and desires. Services includes: Service Coordination; In-Home Support Services; Environmental Modifications; One Sky Family Services Transition Coordinator; and Respite Care

PathWays provides supports for children with disabilities, adults with developmental disabilities or brain injuries in everyday life to maintain their independence and well-being is what we do here at Pathways. Services includes: Service Coordination, Community Support Services, Enhanced Family Care, Community Participation Services and Residential Supports.

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition. Services may include: Information and referral to community services; provide educational advocacy and support, Assistance in planning for transitions; Access to training; Assistance with future planning; Use of Lending Library; Information about acquiring benefits; Referral to appropriate support programs; Assistance to families with the special education process

Service Coordination: We offer case management support to individuals with intellectual disabilities and brain acquired injuries. We advocate and oversee supports and services to ensure continuity of care. Foster Home Services-including recruitment, training, and licensing of foster homes FOR CHILDREN AND FAMILIES WORKING WITH DCYF: Support services for children and families where there has been abuse/neglect, CHINS, or delinquency Placement services for children in licensed facilities when ordered by the court Adoption services including adoptive home studies and placement of children released for adoption

This includes young adults that previously declined this option at the time of their 18th birthday or started the program and discontinued participation.
