Individuals that qualify for the Choices for Independence (CFI) waiver may choose Community Crossroads for their Case Management. Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. Typical services may include but are certainly not limited to: Personal Care; Home Health Aides; Homemakers; Nursing; Meals on Wheels; Adult Medical Day Services; Personal Emergency Response Systems.

Our early learning centers benefit from qualified early childhood teachers with decades of experience in the field and a strong understanding of and passion for early childhood development. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. Nursing services also include medication administration, and treatments; Assistance with personal care including bathing, toileting, and eating; Some transportation options available within service area. Comprehensive Outpatient Community Mental Health Services for Adults and Children. Provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay.

Home Assist Program provides non-medical help to older adults. Examples of help offered: meal planning, grocery shopping, errands; light housekeeping, light laundry; medication reminders, personal care, and companionship.

The Farmington Parks and Recreation Department provides recreational opportunities and special events for the community. Some of the programs offered by the department include: after school program for grades 1-5, free after hours programs for grades 5-8, toddler classes, senior bingo and trip club, summer camp, vacation camp, karate, and many other year round activities. Special events include our annual egg hunt, trick or treat parade, family nights at Fernald park, haunted house, and Hay Day.

For birth families we offer counseling and support during the pregnancy and adoption process. For prospective adoptive families we offer home study and post placement services as well as a domestic adoption program matching adoptive parents with infants in need of a forever family

Guardianship, conservatorship and representative payee services offered statewide. Guardianship Services administers Special Needs Trust. Guardianship Services can provide consultation to families, nursing homes, attorneys and hospitals on all aspects of guardianship.
