CMCC case managers work with individuals and families to promote independent living through person-centered, cost-effective service plans. Case managers monitor services and resolve concerns to ensure quality. We advocate for clients in need of resources (e.g., home modifications, medical equipment, in-home services) to ensure independence, safety, and quality of life.

Adults with disabilities and older adults enrolled in Choices for Independence (CFI) Medicaid waiver; Adults with developmental disabilities enrolled in Developmental Disabilities (DD) Medicaid waiver.
