Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

The Waypoint NH Home Care program provides assistance with homemaking, shopping, medication pre-fills and medication reminders as well as companionship, bathing, dressing, and mobility asistance for seniors, and individuals with illnesses or disabilities. Additionally, extensive big chore services and supervised transportation to medical appointments are provided. A registered nurse or qualified member of the supervisory team will make a free in-home assessment to discuss the needs and develop a personalized plan of care.

Help with managing a chronic illness, support to improve healing and recovery, home safety and falls risk assessment, assistance in recovering from an illness or injury, specialized programs to address the needs of all ages, pediatric and maternal child health services, and management of medications and reconciliation. We provide services for people of all ages at home, in assisted living facilities, and retirement communities.

Early supports and Services (physical therapy, occupational therapy, speech/language therapy and early childhool special education) for children birth to age three. Additionally, we offer services for parents and caregivers, pediatric out patient services and therapeutic play groups.

Family Centered Early Supports and Services (FCESS) is a Federal and State mandated program that provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay. Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

Registered Nurses: Working under physician’s orders, perform health assessments, administer and monitor medications, dressing changes - wound care, injections, health teaching and assist in the management of illness in the home. Home Health Aides: Caring for personal hygienic needs under supervision of registered nurses.

It helps patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home. Personal Care Services assists with daily tasks, such as bathing, grooming, light housekeeping and meal preparation, so people can continue to live in their homes independently.

We partner with families to meet the developmental needs of the child for children birth to age 3. For those children found eligible, we offer services that include parent coaching, physical, occupational, speech and developmental therapies.

1. Skilled in home nursing services offered include: physical, occupational, speech therapy; IV drugs, shots, tube feedings, dressing changes, and more. 2. Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. Hospice (nursing care, pain management, patient and family support, etc.

Our goals are to avoid or limit hospitalization, thereby improving a patient’s quality of life, and to help a patient achieve independence in his/her own care.

Home Health Aide and Homemaker Services. Part-time care for convalescents or handicapped people may include meal preparation, light housekeeping, shopping, personal, and health care. Hospice Program. In addition to 24-hour availability of a visiting nurse, this program provides trained volunteers to counsel terminally-ill patients and their families during the illness and in the bereavement period. Children's Health Care: Child Health Clinics in all areas provide complete health care for low-income children. Immunization and preventative health education for preschool children are provided in home and clinic settings.
