Ascentria Care Alliance is a Licensed Home Health Care provider and certified as an Other Qualified Agency (OQA). That means that they work with a variety of payment options whether the recipient plans to pay for services privately, qualifies for Medicaid and participates in Choices for Independence waiver (HCBC), has long term care insurance or has a grant. Registered Nurses: Working under physician’s orders, perform health assessments, administer and monitor medications, dressing changes - wound care, injections, health teaching and assist in the management of illness in the home. The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

Easter Seals' statewide Caring Companions Program provides non-medical home care services for: older adults, people with a chronic illness, and people with disabilities.

With help of Early Supports and Services, many parents and caregivers have navigated the uncertainties and questions of the early years when they have a child who is showing a delay. We work with families to prevent, identify, assess, and treat children with developmental delays.

Family Centered Early Supports and Services (FCESS) is a Federal and State mandated program that provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay. Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. Typical services may include but are certainly not limited to: Personal Care; Home Health Aides; Homemakers; Nursing; Meals on Wheels; Adult Medical Day Services; Personal Emergency Response Systems.
