The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

Registered Nurses: Working under physician’s orders, perform health assessments, administer and monitor medications, dressing changes - wound care, injections, health teaching and assist in the management of illness in the home. Home Health Aides: Caring for personal hygienic needs under supervision of registered nurses.

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. The Waypoint NH Home Care program provides assistance with homemaking, shopping, medication pre-fills and medication reminders as well as companionship, bathing, dressing, and mobility asistance for seniors, and individuals with illnesses or disabilities. Additionally, extensive big chore services and supervised transportation to medical appointments are provided. A registered nurse or qualified member of the supervisory team will make a free in-home assessment to discuss the needs and develop a personalized plan of care.

Help with managing a chronic illness, support to improve healing and recovery, home safety and falls risk assessment, assistance in recovering from an illness or injury, specialized programs to address the needs of all ages, pediatric and maternal child health services, and management of medications and reconciliation. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

Individuals that qualify for the Choices for Independence (CFI) waiver may choose Community Crossroads for their Case Management. Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. Typical services may include but are certainly not limited to: Personal Care; Home Health Aides; Homemakers; Nursing; Meals on Wheels; Adult Medical Day Services; Personal Emergency Response Systems.

Family Centered Early Supports and Services (FCESS) is a Federal and State mandated program that provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay. Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

In addition to 24-hour availability of a visiting nurse, this program provides trained volunteers to counsel terminally-ill patients and their families during the illness and in the bereavement period. Children's Health Care: Child Health Clinics in all areas provide complete health care for low-income children. Immunization and preventative health education for preschool children are provided in home and clinic settings.
