SPECIALIZED TREATMENT | VNA OF MANCHESTER & SOUTHERN NH, INC.
Our Home Care Program provides high-quality, cost effective alternatives to institutional care. A team of nurses, therapists, social workers, aides and dietitians work under the order of a physician to provide patients and their families with essential healthcare services in the comfort of their home. In developing an appropriate plan of care for each patient, staff are trained to utilize the varied expertise of the home care team to provide the maximum care necessary for recovery. This team works together with the patient and family to promote independence. The home care team assists in making those individuals who are recovering from surgery or a serious illness feel more comfortable and safe by providing comprehensive care in the home. A registered dietitian is available for nutritional counseling, and a registered nurse is on call 24 hours a day.
