The Ways2Wellness CONNECT Program links Community Health Workers (CHW)s with patients who are struggling with unmanaged chronic disease and experiencing barriers to achieving overall health and wellness to enhance quality of life. The program currently works with patients ages 55 and over with unmanaged chronic disease, such as hypertension, diabetes, congestive heart failure, COPD, etc. at no cost to the patient or provider.

Exeter Hospital offers Palliative care. This hospital has a partnership with Massachusetts General Hospital Cancer Center to provide specialized inpatient and outpatient cancer care. Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

The New Hampshire Colorectal Cancer Screening Program (NHCRCSP) is a statewide effort to increase colorectal cancer screening for New Hampshire residents. The program will also provide a limited number of free colonoscopies to men and women who are New Hampshire residents and who meet financial and clinical eligibility for the screening program.

Lamprey Health Care is a non-profit health care clinic. The clinic provides a wide range of primary health care services to people of all ages. Other services provided are counseling, HIV testing, chronic disease management, physicals, wellness programs, diabetes screening and education, and more. A Breast and Cervical Cancer Program (BCCP) screening site for the State of NH. Provides free breast and cervical cancer screening to eligible patients. We have a nurse who assists patients that may be eligible to receive free breast and cervical cancer screenings if they are uninsured. An Amoskeag Health Nurse provides education, support, and assistance to navigate through the program’s options.

Family Medicine focuses on preventing illness or injury through regular checkups, health screenings and immunizations, and other strategies.

The Family Health Center and its satellite, Hillsboro-Deering Family Health provide primary and preventive care for the entire family in collaboration with NH-Dartmouth Family Practice Residency. As a state-supported community health center, the health centers provide a full range of primary care services regardless of ability to pay. Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services
