Mobile health teams bring medical and social work services to places like shelters, soup kitchens and public housing. Services provided at clinics include preventive health screenings; sick visits; and chronic-disease management. The team will also link patients with staff at Families First and Goodwin Community Health who can help with eligibility determination and applications for housing assistance, food stamps, WIC, TANF, disability benefits, Medicare, Medicaid, other health insurance, veterans benefits and more. Clinic staff can also help patients access substance misuse and mental health counseling, family support, specialty care, medications, and labs and diagnostic testing.
** As of September 2024, clinics were offered in Dover, Rochester, Portsmouth and Exeter. Clinic locations are subject to change.
