Birthright provides referrals for medical, legal, housing, social services, counseling, education, and many other topics as related to pregnancy.

The program helps participants find various health and wellness services quickly and easily. By serving as an access point to these resources, the MOM program helps new and expecting parents and their babies have good health during pregnancy and beyond. Services include: Medical, behavioral health, and substance use screening Care coordination and case management, including housing assistance, service connection and collaboration, and food assistance Behavioral health services Substance use treatment, including outpatient, inpatient and residential, and recovery supports and resources Perinatal and postpartum care, including OB/GYN connections, check-ups, birthing classes, midwife services, and lactation support Family planning Nutrition services Medication Assisted Treatment (MAT) services Peer support services

The WIC program provides supplemental foods, health care referrals, and nutrition education at no cost to the families of low-income pregnant, breastfeeding and non-breastfeeding post-partum women, and to infants and children up to 5 years of age.

Our home visiting programs help families by providing support and knowledge in a wide variety of areas. Each home visitor respectfully supports families in their roles as their child's first and best teacher while teaching families how to help their child/children grow and learn in a healthy home environment. TLC Family Resource Center nurses and parent educators provide comprehensive family support services including home visits, case management, health education, parent education, and supervised visitation services. The agency has a sexual health education program for young women and men ages 16-21.

Provides information to reduce the incidence of injury death & disability through prevention and intervention. Extensive resources regarding seat belts & car safety seats, fire safety, drowning prevention, playground safety, home injury prevention, bicycle safety, firearm safety, etc. Also maintains a list of resources for obtaining a bike helmet for children at a low cost.

Harbor Care often prioritizes its housing for veterans and individuals and families living with disabilities, including behavioral health disorders such as mental illness or substance abuse disorders, physical disabilities, or those living with chronic illness such as HIV/ AIDS. Our housing comes in a variety of forms - from shared houses to apartment complexes to "vouchers" that allow a client to choose their own home within the community. In all cases, rent remains affordable, with clients paying a portion of their income towards rent. Our housing often comes bundled together with services such as case management, to help clients find employment, obtain entitlements and benefits for them and their families, and avoid returning to homelessness. Programs include: --Permanent Supportive Housing Program, for chronically homeless individuals through federal subsidies (non-voucher). --Mainstream Voucher Program, for chronically homeless individuals through a federal voucher. --Boulder Point, a project-based HUD-VASH (Veterans Affairs Supportive Housing) voucher program for veterans. --Harbor Homes and Harbor Care also operate other federally-subsidized properties, which helps us offer lower rents to qualified community members. Outpatient Program and Intensive Outpatient Program (IOP) for women who experience substance use and a history of trauma and/or mental health issues. The program includes: case management, group treatment; and individual counseling. Program length averages 16 weeks. Childcare provided as needed. Transportation provided if needed within the city of Manchester. *****This program is open to community members as well as to Families in Transition residents.******
