PMCC assists the Elderly and Disabled Adults when applying for Long Term Care and Choices for Independence (CFI) Medicaid waiver. We work with the families of individuals who are unable to go out into the community due to their poor health. We bridge the gaps of communication between our clients, with the Nursing Homes and Assisted Living Facilities, and with the State of NH to process these applications.
