Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

Transitional housing services where individuals with limited community placement options live in a residential setting with appropriate treatment.

Critical Time Intervention (CTI) is a no-cost, intensive care transition program connecting people to services and supports in their home communities upon discharge for a psychiatric inpatient setting.

Mental Health peer support and services for adults ages 18 and older, who are in crisis or who are having a rough time and need a safe space with 24/7 support.

Riverbend offers a continuum of residential options for people with psychiatric disabilities, including supervised apartments, short-term crisis and transitional housing, group homes, specialized services for elders and long-term residential care.

Long-term communal housing for individuals living with severe and persistent mental illness who would otherwise be homeless or institutionalized

Offering behavioral health services to clients, and then pairing these same clients with clinical teams that can simultaneously address co-occurring health care problems, chronic diseases and/or substance misuse disorders.

The Housing Transition Program is a subsidy program for those leaving NH Hospital or other inpatient psychiatric program to private sector housing. The program operates in the same manner as Section 8 but is for individuals with mental illness to bridge the gap between the time they exit a mental health facility and the time they obtain Section 8. Participants must get on the Section 8 waitlist.
