The Aging Resource Center provides older adults, families, and community members with information, education, and support to help them live healthier and more informed lives.

Diabetes Care services available include: individual and group education and training, insulin pump and continuous glucose monitoring programs, the diabetes fitness program, and a diabetes support group.

Breast and cervical cancer screenings Program site of the Let No Woman Be Overlooked Breast and Cervical Cancer Program, a program of the NH Dept. of Health & Human Services that provides free breast and cervical cancer screenings to women who are between 21 & 64 years old, have no health insurance coverage for these tests, and meet income guidelines.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Harbor Care is uniquely designed to support your recovery journey-with or without insurance. No one is turned away, regardless of ability to pay. A full range of quality residential and outpatient services are offered so that we can accompany clients on their journeys from the first time they consider treatment to long-term recovery. We offer: outpatient treatment, including our High Intensity Outpatient Program (PHP and IOP); treatment for pregnant and parenting women and their children at the Cynthia Day Family Center; Medication Assisted Treatment (MAT) for opioid and/or alcohol dependence; long-term recovery support counseling and groups statewide with our Peer Recovery Support Services; and services for the Rockingham County Drug Court. We're dedicated to partnering with schools to ensure the health, safety, and well-being of its staff, student body and their families. We work on-site and through telehealth, side-by-side with school social workers, nurses, teachers and administrators to being health care and supportive services not just to the students who need it most, but also students' families and school faculty. We're with our community all of the way! We help youth and their loved ones access specialty care providers and social services, often working with our school nurse partners. We also connect families with systems and ensure long-term supports are in place for students with behavioral health issues. With specialized treatment that may include child care and transportation for parents in need of behavioral health care, we take a holistic, whole-family approach to ensuring health and wellness.

Free Primary care, smoking cessation, wellness, diabetes and asthma clinics. Care is provided by volunteer physicians and includes: dermatology, rheumatology, psychiatry, gastroenterology, women's health, endocrinology, primary care and neurology.

Numerous support groups and programs are offered by the American Cancer Society. The Reach to Recovery program helps individuals with breast cancer by providing one-on-one peer support. This program matches a breast cancer survivor with a recently diagnosed patient who has similar issues.

Your oncology nurse navigator is your advocate, reinforcing and supporting the physician/patient relationship. Oncology nurse navigators help newly diagnosed cancer patients, current cancer patients and family members. Your oncology nurse navigator will: Facilitate communication between all members of the healthcare team Provide education on disease processes, treatment options and clinical trial availability Help patients make informed decisions about their care Coordinate office visits across medical disciplines to streamline cancer care delivery Serve as a reliable and trusted source with answers to any questions Organize multidisciplinary team conferences to promote collaboration in cancer treatment Provide a personal touch to cancer care by visiting patients at appointments, assessing patients’ needs and relaying relevant information to providers Advocate for patients’ needs and link them with cancer support resources

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

Diabetes Self-Management Program is a comprehensive outpatient referral-based education program that empowers participants to take control and successfully manage their diabetes. Participation provides you with the opportunity to fully understand the rationale for early aggressive intervention to prevent complications from diabetes. The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.
