Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

We provide comprehensive virtual care for adults with Pre-diabetes, Type 2 diabetes, Obesity, Hypertension, and/or High Cholesterol. Patients can track their blood sugar levels, medication adherence, and symptoms at home, while their clinician can review the data, adjust treatment plans, and offer personalized advice remotely. All care is coordinated and communicated directly to the patients primary care team. We are an extension of the primary care team and partner with both the patient and primary clinician to help you manage your health. Services include: - Weekly, biweekly, monthly, or quarterly telehealth visits tailored to your individual needs. - Medication and Insulin Management. Includes prior-authorization handling for related medications. - Continuous glucose monitoring. - Nutrition & lifestyle counseling. - Remote patient monitoring (Blood sugar, Blood pressure) - Chronic care management. - Coordination of Diabetic Retinopathy Screenings & Diabetic Foot Exams. - Limited availability for home visits - determined on a case by case basis.

Harbor Care is uniquely designed to support your recovery journey-with or without insurance. No one is turned away, regardless of ability to pay. A full range of quality residential and outpatient services are offered so that we can accompany clients on their journeys from the first time they consider treatment to long-term recovery. We offer: outpatient treatment, including our High Intensity Outpatient Program (PHP and IOP); treatment for pregnant and parenting women and their children at the Cynthia Day Family Center; Medication Assisted Treatment (MAT) for opioid and/or alcohol dependence; long-term recovery support counseling and groups statewide with our Peer Recovery Support Services; and services for the Rockingham County Drug Court. We're dedicated to partnering with schools to ensure the health, safety, and well-being of its staff, student body and their families. We work on-site and through telehealth, side-by-side with school social workers, nurses, teachers and administrators to being health care and supportive services not just to the students who need it most, but also students' families and school faculty. We're with our community all of the way! We help youth and their loved ones access specialty care providers and social services, often working with our school nurse partners. We also connect families with systems and ensure long-term supports are in place for students with behavioral health issues. With specialized treatment that may include child care and transportation for parents in need of behavioral health care, we take a holistic, whole-family approach to ensuring health and wellness.
