The Community Integration Program (CIP) includes comprehensive wrap-around services available 24 hours per day, 7 days per week, that vary in support and intensity based on the participant’s needs. Services are designed to support individuals in building and sustaining skills for successful independent living in the community. Our philosophy is based on principles of the Assertive Community Treatment (ACT) model, an evidence-based practice that is designed specifically for individuals with serious mental illness with or without co-occurring substance use disorders. Some of these principles include embracing a holistic approach to treatment, providing integrated services by one team, promoting continuity of care, and building upon natural supports to successfully maintain community living.
