Our staff members provide rehabilitative services in natural community environments, helping to reinforce and enhance the client’s ability to successfully manage the symptoms of mental illness and achieve their personal and treatment goals. The intent is to help the client avoid hospitalization and gain confidence throughout their recovery.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

REAP is a prevention program that is available to all older adults in the community. The program offers free, confidential counseling and educational services to help participants better understand and deal with the many life changes they encounter.
