REAP is a prevention program that is available to all older adults in the community. It is designed to help you take control of your life, to live a happy, healthy and independent lifestyle. They offer up to 6 free, confidential counseling and educational services to help you better understand and deal with the many changes you encounter.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

Comprehensive Outpatient Community Mental Health Services.

Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. The Brattleboro Retreat's Adolescent Inpatient Program offers crisis stabilization, specialized treatment, and aftercare planning for young people ages 12 to 18.

Mental health care for children and adolescents and their families. Counseling and medication management to address issues including depression, anxiety, trauma, and abuse.

The Plymouth House is a 12-Step retreat and addiction treatment center nestled in the picturesque foothills of the White Mountains in New Hampshire. The Plymouth House has combined its expertise as a successful 12-Step retreat with Guardian’s world-class medical and clinical care.

General counseling services for individuals and families

Neurobehavioral Residential/Educational Treatment Program at the Zachary Road facility designed for males and females from ages 5 to 21 with pervasive developmental disorders, nonverbal learning disabilities, traumatic brain injuries, and other neurological disorders. Services includes: on-site psychiatry, nursing, psychology, education, nutrition, recreation therapy, teaching of independent living skills, vocational training, clinical social work, speech and language and occupational and physical therapy evaluation and services.

Behavioral health services covers all mental health issues, including treatment for addiction, behavior modification, and problem solving therapy. All services are integrated with the primary care provider to ensure comprehensive case management.

Comprehensive mental health care for adults and children, including medication management. Counseling can be in an individual, couples, or family setting.

Lakes Region Mental Health Center provides a wide range of behavioral health services to children, adolescents, young adults and their families. In some cases, it is important to include families in the treatment plan whenever possible. Services includes: Individual, Group Family Therapy; Case Management; Functional Support Services; Acute Care Services; Mobile Crisis Response Team; Child to Adult Transition; Trauma Focused Cognitive Behavioral Therapy; Dialectical Behavioral Therapy; Art Therapy

Counselors help patients manage health conditions including anxiety, depression, life transitions, substance misuse, diabetes and other chronic illnesses. They also support patients in making changes such as improving diet and exercise habits or reducing substance misuse. NOTE: Must be an established patient at Families First Health Center or Goodwin Community Health in order to use these services.

The Retreat's Abigail Rockwell Children's Center (ARCC) is a multi-track residential treatment center providing expert care for children ages 6 to 14. Located in a home-style dwelling that accommodates 8 children, this setting is ideal for specialized programming to help children who are transitioning from childhood to adolescence.

The counselors on staff at HealthFirst are available to meet with you confidentially regarding individual, couple or family issues. Our licensed clinical social workers and psychologists offer services as an integral part of the HealthFirst Patient Centered Medical Home approach. Provides people with the resources and support needed to manage their substance abuse disorder.
