Volunteers provide friendly visits, light chores, and grocery shopping. Telephone reassurance and SMALL home repairs are also available to existing clients. *Services subject to volunteer availability. **This agency does not help with heater repairs or any large repair services. This program is nonfunded and all services are provided by volunteers.

Individuals that qualify for the Choices for Independence (CFI) waiver may choose Community Crossroads for their Case Management. Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. Typical services may include but are certainly not limited to: Personal Care; Home Health Aides; Homemakers; Nursing; Meals on Wheels; Adult Medical Day Services; Personal Emergency Response Systems.

The CCL Help line is a confidential service that opens access to local volunteers and partner organizations who can help you or a loved one. We can refer you to local resources that can help with transportation, home repair and safety improvements, shopping, meals, household help, gardening and lawn care, personal care, companionship, pet care and legal and financial management.

Home Health Care includes skilled nursing care and physical, occupational and speech therapies. It helps patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home. Personal Care Services assists with daily tasks, such as bathing, grooming, light housekeeping and meal preparation, so people can continue to live in their homes independently.

Home Assist Program provides non-medical help to older adults. Examples of help offered: meal planning, grocery shopping, errands; light housekeeping, light laundry; medication reminders, personal care, and companionship.

Cleaning for a Reason provides free housecleaning services to any woman, man, or child in active cancer treatment. Cleaning for a Reason partners with maid services, including some maid services in NH. To apply for services go to cleaningforareason.org, Patients tab. This is not a guaranteed service. Program is zip code and capacity based.

Gateways for Seniors provides in-home nursing services, personal care and homemaking services to older adults and adults who are chronically ill. Some of the homemaker services provided are: meal preparation, light housekeeping, laundry, bill paying, transportation, etc. Some of the nursing services provided are: medication management and administration, health monitoring, diabetic care, case mangement, colostomy care, wound care, catheter care, dressing changes, post surgery care, etc. The agency can also provide help with personal hygiene. These services provide respite for families and caregivers.

Gateways CFI Case Management provides conflict-free case management and is unable to refer CFI clients to Gateways for Seniors home care service program. This program is designed to help adults 18 years or older who have nursing home-level care but prefer to live at home.
