VOLUNTEER CAREGIVING PROGRAM | COMMUNITY CAREGIVERS OF GREATER DERRY & THE LOANERS CLOSET

Volunteers provide friendly visits, light chores, and grocery shopping. Telephone reassurance and SMALL home repairs are also available to existing clients. *Services subject to volunteer availability. **This agency does not help with heater repairs or any large repair services. Individuals that qualify for the Choices for Independence (CFI) waiver may choose Community Crossroads for their Case Management. Case Managers are responsible to establish services, meet regularly with each person and advocate for all services critical to their medical care needs to support them in their desire to remain in their homes and out of nursing care facilities. GATEWAYS FOR SENIORS | GATEWAYS COMMUNITY SERVICES

Gateways for Seniors provides in-home nursing services, personal care and homemaking services to older adults and adults who are chronically ill. Some of the homemaker services provided are: meal preparation, light housekeeping, laundry, bill paying, transportation, etc. Some of the nursing services provided are: medication management and administration, health monitoring, diabetic care, case mangement, colostomy care, wound care, catheter care, dressing changes, post surgery care, etc. The agency can also provide help with personal hygiene. These services provide respite for families and caregivers.
