Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

Provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay.

Services includes: Home visits for treatment, Safety checks, Emotional support, Hospice care, Diet and exercise counseling, Creation of living wills, DNR’s and health care proxies, Advocating for patients and their families

Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. We are ready to offer a home health care solution tailored to meet your needs and lifestyle. Help with Bathing, dressing, toileting, grooming, housekeeping and cooking, eating mobility assistance, errands and shopping medication reminding.

Easter Seals' statewide Caring Companions Program provides non-medical home care services for: older adults, people with a chronic illness, and people with disabilities.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

The Personal Care Attendant (PCA) Program is a self-directed program which allows individuals real choice and control over their personal care services. Our experienced Nurse Coordinators help individuals design programs tailored to their needs and lifestyles, and teach them to successfully manage their personal care attendants and overall program. This program is funded by New Hampshire Medicaid.

The Waypoint NH Home Care program provides assistance with homemaking, shopping, medication pre-fills and medication reminders as well as companionship, bathing, dressing, and mobility asistance for seniors, and individuals with illnesses or disabilities. Additionally, extensive big chore services and supervised transportation to medical appointments are provided. A registered nurse or qualified member of the supervisory team will make a free in-home assessment to discuss the needs and develop a personalized plan of care.

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

Help with managing a chronic illness, support to improve healing and recovery, home safety and falls risk assessment, assistance in recovering from an illness or injury, specialized programs to address the needs of all ages, pediatric and maternal child health services, and management of medications and reconciliation. For those who choose to die in the comfort and dignity of their own surroundings, Hospice at HCS provides comprehensive care and support to those with a life limiting diagnosis. Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers, home health aides, spiritual care counselors and bereavement counselors.

Early supports and Services (physical therapy, occupational therapy, speech/language therapy and early childhool special education) for children birth to age three. Additionally, we offer services for parents and caregivers, pediatric out patient services and therapeutic play groups.

Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

Registered Nurses: Working under physician’s orders, perform health assessments, administer and monitor medications, dressing changes - wound care, injections, health teaching and assist in the management of illness in the home. Home Health Aides: Caring for personal hygienic needs under supervision of registered nurses.

Home Health Care includes skilled nursing care and physical, occupational and speech therapies. It helps patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home. Personal Care Services assists with daily tasks, such as bathing, grooming, light housekeeping and meal preparation, so people can continue to live in their homes independently.
