Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers home health aides, spiritual care counselors and bereavement counselors.

Hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible.

Our Personal Care Service Program is a self directed program which provides non-medical hands-on assistance with activities of daily living for people with disabilities. Some of our services include, but are not limited to: personal care and homemaking services; respite care; housekeeping and laundry; grocery shopping and errands; meal planning and preparation; personal hygiene and medication reminders. The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

Family Centered Early Supports and Services (FCESS) is a Federal and State mandated program that provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay. Services are typically provided in the home (however can be provided in other natural settings as requested by family) and in a transdisciplinary fashion. A team of early childhood professionals including licensed educators, occupational therapists, physical therapists, speech language pathologists, and other specialized consultants work in conjunction with the eligible child and family to help the child develop skills to his/her full potential as well as provide the family with strategies to directly support and reinforce their child’s development.

The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

Registered Nurses: Working under physician’s orders, perform health assessments, administer and monitor medications, dressing changes - wound care, injections, health teaching and assist in the management of illness in the home. Home Health Aides: Caring for personal hygienic needs under supervision of registered nurses.
