Comprehensive Outpatient Community Mental Health Services for Adults and Children. Services may include: Case Management, Outpatient Therapy, Psychiatric Services, Functional Supports and Services, Supported Employment Services, Assertive Community Treatment (ACT), Illness Management and Recovery, Financial Case Management, Infant/Early Childhood Mental Health, Parenting Education

CLM provides array of mental health care for adults, children and their families.

• Children Services: Our specialists work with children experiencing serious emotional difficulties such as depression, attention deficit and hyperactivity disorder, autism, disruptive behaviors, substance use disorders, and trauma associated with emotional, physical or sexual abuse.
• Adult Community Support provides a multi-faceted range of services with a goal of supporting restoration, rehabilitation and recovery for clients with severe and persistent mental illnesses such as schizophrenia, mood and depressive disorders.

Services includes: Individual and Group Therapy, Targeted Case Management, Medication Services, Evidence Based Practices, Evaluation and Treatment of Substance Use Disorders, Functional Support Services, Wellness Works, Assertive Community Treatment (ACT), Supported Housing and Homeless Outreach, Critical Time Intervention.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Services for adults and children with developmental disabilities and/or acquired brain injuries include case management, day programs, employment programs, family support and residential services. Some of the supports and services available (ages 3 to 20) include: Autism Supports; In Home Supports; Parent-to-Parent Program; High School Transition; and Futures Planning. Services include: Information and referral to services and resources; Respite Services; Respitality Programs; Recreation and Summer Camps; Funding assistance and variety of assistive technology products.

Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. Additional Services may include: Service Coordination, Respite, High School Transition Supports, In Home Support Services, Personal Care, Mobility and Physical Functioning, Consultation and Education, Functional Supports and Services.

One Sky offers a variety of support, day and residential services for children and adults with developmental disabilities and traumatic/acquired brain disorders. Each service is individually tailored in conjunction with a person's circumstances, needs and desires. Services includes: Service Coordination; In-Home Support Services; Environmental Modifications; One Sky Family Services Transition Coordinator; and Respite Care

The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Community Bridges offers a wide range of individualized services to meet the needs of people with disabilities. This includes programs such as Case Management to identify community resources. Participant-Directed and Managed Services offer In-Home Support and Self Directed services. Day and residential services for adults provides shared and staffed homes as well as community-based activities including employment support.

The Life Skills Workshop can help change the course of your life! You can reduce much of the turmoil in your life and the lives of those around you by making solid choices. The Life Skills Work-shop can help you learn skills and strategies for things like record keeping, voting, dealing with conflict, and more!

We work with individuals age 3-21 that experience a developmental disability and live with a family member. We offer support at education meetings, guide you to community resources, and help with the transition into adult life. Children's In Home Supports - An intensive home based program helping children with complex medical and behavioral needs. Services that are offered are personal care, service coordination, consultative services (including behavioral), respite care, and environmental modifications.

In person and telehealth mental health services for ages 3 and older. Services include, but not limited to: individual, couples, group, family therapy and Psychological and Neuropsychological assessments and testing. Support services for established clients may include: case management, vocational, community, and home based services.

Independent Living programs can Help overcome barriers; especially those that impede a person's Ability to pursue activities of his/her own choice.

Service Coordination: We offer case management support to individuals with intellectual disabilities and brain acquired injuries. We advocate and oversee supports and services to ensure continuity of care.
