Cornerstone VNA is a non-profit home, health and hospice agency currently serving Rockingham, Strafford, Belknap and Carroll Counties in NH and York County in Maine. The team at the VNA uses the latest technology to provide the most highly skilled nursing, rehabilitative therapies, social work, and support services in their service area. Certified Specialty programs include Wound & Ostomy Care, Mental Health Nursing, Intravenous Therapy, Chronic Care Management, Hospice & Palliative Care, Pain Management and Telehealth services for patients of all ages.

Our Community nurse works with you, your family, and other health professionals and caregivers to provide coordinated health care that addresses your physical, emotional, and social needs. Our goal is to help Hanover residents live safely and as independently as possible at home and in their community. There is no fee for these services, which include: Coordinating your care with your primary physician/agencies/resources; Providing health education, resource information and referrals; Serving as your advocate for your medical needs; Making home visits to assess your current health care needs; Assisting in scheduling medical appointments and helping ensure you can attend and effectively participate in visits with health care providers; Assisting in helping you obtain medical equipment; Assisting with questions about insurance, advance directives and long-term care planning.

1. Skilled in home nursing services offered include: physical, occupational, speech therapy; IV drugs, shots, tube feedings, dressing changes, and more. 2. Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. The Hospice House is a home that includes 24-hour skilled nursing care for terminally ill patients and their families. Some of the amenities of the Hospice House include a pull-out sofa bed in each room for family and friends to sleep, a personalized menu, a family kitchen area, and a Multi-Faith Meditation Room. There is also access to a computer for e-mail and Internet, and an outside walking path and garden area.

Provides "wishes" for NH children between the ages of 3-18 with a serious or chronic illness. Referrals must come from an agency or person other than a family member. Eligibility guidelines apply.

Mercy Medical Angels has provided free transportation to medical care since 1972 ensuring that no one in need is denied medical care because they don't have transportation. Ground transportation is provided with gas cards, bus and train tickets. Air transportation is flown by volunteer pilots or a commercial airline partner.

Certified Application Counselors are available to assist patients with information on the various assistance programs available. They are also available to assist with the application process. Individuals do not need to be patients of Lamprey Health Care to meet with a Certified Application Counselor.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

Free temporary lodging for families of children who are receiving care at the Dartmouth Health Children’s in Lebanon, NH. Lodging is dependent on space available. Reservations are not taken. Names are put on a waiting list. David's House will do everything possible to accommodate your needs or work with you to find alternative arrangements in the event that the house is full. The length of stay is for as long as the family has a hospital-related need for local lodging. Food is also available along with free toiletries and laundry facilities.

Palliative care can help patients of all ages during and after curative treatment at any stage of their illness. The goal is to improve quality of life while considering a patient’s unique needs. Our palliative care core team includes a palliative care physician and/or nurse practitioner, nurse, social worker, and spiritual counselor.
