Services are provided in conjunction with a patient's physician. A doctor's order is required for eligibility. Patients must be homebound and have a need for skilled healthcare and supportive care. Agency staff will evaluate eligibility via phone or home visit.

Referrals can be made 24 hrs/day, 7 days a week. Call before faxing evenings or weekends.

Fees

The VNA & Hospice of VT and NH serves all who qualify, regardless of age or ability to pay. Services are paid through Medicare, NH/VT Medicaid and private insurance companies.
