The Family Wellness Home Visiting program helps at-risk-families, focusing on prevention by offering emotional support and practical solutions to managing family life as well as in home assistance with parenting skills and household management and promoting self-sufficiency by coaching families to be able to access community services and advocate for themselves. Helping to access employment programs, addiction and recovery programs, attend medical/social service appointments, school meetings and recreational activities.

Our Community nurse works with you, your family, and other health professionals and caregivers to provide coordinated health care that addresses your physical, emotional, and social needs. Our goal is to help Hanover residents live safely and as independently as possible at home and in their community. There is no fee for these services, which include: Coordinating your care with your primary physician/agencies/resources; Providing health education, resource information and referrals; Serving as your advocate for your medical needs; Making home visits to assess your current health care needs; Assisting in scheduling medical appointments and helping ensure you can attend and effectively participate in visits with health care providers; Assisting in helping you obtain medical equipment; Assisting with questions about insurance, advance directives and long-term care planning.

Wellness Connection offers an array of assessment, education, exercise and rehabilitation services to help you stay well, manage existing health conditions and prevent new problems. Physician Referral Service includes detailed information about professionals: their office locations and hours, academic training and credentials.

Our Portsmouth Health Care Center offers primary care to keep our New Hampshire communities healthy. This facility conveniently offers on-site pharmacy, lab, and radiology services, as well as physical therapy and health education courses.

Living Well with Chronic Pain: Living and coping with a chronic condition and/or chronic pain can be very challenging. We offer evidence-based self-management programs designed to give you the skills, tools and confidence you need to help take control of your chronic condition(s) and live life to the fullest. Caregivers and loved ones of those who manage chronic conditions or pain are also encouraged to participate. What is covered in these programs?  - Condition related problem solving - Communication skills - Managing emotions - Physical Activity - Strategies to deal with fatigue, pain, frustration, isolation and poor sleep - Developing patient/physician partnership - Use of community resources - Nutrition - Managing medications and how to evaluate new treatments  - Cognitive symptom management - Benefit from the support and encouragement from others also living with chronic disease or pain 

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.
