Concentra offers occupational and urgent medical care, as well as physical therapy and wellness services. We treat everything from sprains and broken bones to coughs, colds, and flu.

REAP is a prevention program that is available to all older adults in the community. Confidential counseling and educational services to help participants better understand and deal with the many life changes they encounter. Topics Includes: Stress; Relationships; Grief; Emotional wellbeing; Substance use; Family dynamics and Quality of life

The Mobile Health Service provides critical health-related services out into the community.

Community clinic offering a full range of primary and speciality health care services. Mental health services are available. Physicals offered. -Serves people of all ages. -Medical social workers help link patients to additional services and community help, including referrals to dentists accepting Medicaid. -Transportation to medical appointments can be arranged.

Mid-State Health Center is an independent, non-profit, Federally Qualified Health Center with offices in Bristol and Plymouth, NH. A full range of outpatient primary care services including substance use disorder treatment.

Our Community nurse works with you, your family, and other health professionals and caregivers to provide coordinated health care that addresses your physical, emotional, and social needs. Our goal is to help Hanover residents live safely and as independently as possible at home and in their community. There is no fee for these services, which include: Coordinating your care with your primary physician/agencies/resources; Providing health education, resource information and referrals; Serving as your advocate for your medical needs; Making home visits to assess your current health care needs; Assisting in scheduling medical appointments and helping ensure you can attend and effectively participate in visits with health care providers; Assisting in helping you obtain medical equipment; Assisting with questions about insurance, advance directives and long-term care planning.

The Well Child program offers: developmental assessments, health education, parenting education, assistance with applications.

Granite VNA provides community services for infants through seniors in preventative health care, health education and health maintenance. Special programs are available, such as Baby's First Homecoming for new mothers who are discharged from Concord Hospital, and foot care clinics that offer foot care, blood pressure screenings, medication education, and nutritional counseling to seniors.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. Non medical peer support for adults eighteen (18) years and older with a mental illness. Groups, activities, and educational activities are also offered. Transportation for members only when available.
