Our Community nurse works with you, your family, and other health professionals and caregivers to provide coordinated health care that addresses your physical, emotional, and social needs. Our goal is to help Hanover residents live safely and as independently as possible at home and in their community. There is no fee for these services, which include: Coordinating your care with your primary physician/agencies/resources; Providing health education, resource information and referrals; Serving as your advocate for your medical needs; Making home visits to assess your current health care needs; Assisting in scheduling medical appointments and helping ensure you can attend and effectively participate in visits with health care providers; Assisting in helping you obtain medical equipment; Assisting with questions about insurance, advance directives and long-term care planning.

Trained leaders teach and lead attendees in exercises that help build bone density and strength using weight bearing and non-weight bearing exercises. The weights are provided at each session. This low impact program also helps with balance, flexibility, muscle strength, overall health and social relationships. The program helps to rebuild bone density and also helps with balance.

An Asthma Educator will work with parents and children to develop a plan for improved asthma management. Topics covered include information about child's medication; asthma action plans and how they can help your child's asthma; and identifying common asthma triggers in the home. When funds allow, the program will provide you with the items you need to help get your child's asthma under control, such as mattress and pillow case covers, and allergen-free cleaning supplies.
