Concord Hospital and other community healthcare facilities sponsor a variety of support groups that are open to the public.

Purpose is to help the transition from childhood to adulthood, dependence to independence and pediatric medical care to adult medical care go more smoothly. Help teens and young adults to: Get the information they need to live independently; Live a successful life; Make decisions about healthcare; Inform your doctors and physicians about your healthcare and general needs; Make information easily accessible to teens who may lack parental guidance.

Health and wellness education classes offered. A physician finder/referral is offered free of charge.

Free Primary care, smoking cessation, wellness, diabetes and asthma clinics. Care is provided by volunteer physicians and includes: dermatology, rheumatology, psychiatry, gastroenterology, women's health, endocrinology, primary care and neurology.

This is a general acute care hospital. The hospital is open 24/7. It provides emergency room care. It also has a general Intensive Care Unit. The hospital has a Heart and Vascular Institute.

The Mobile Health Service provides critical health-related services out into the community.

Every month, CMC offers numerous free and low cost screenings, classes, and activities including health screenings for early detection of disease, an annual Women's Health Forum and partnerships with the Manchester School District, assisting children in developing healthy lifestyle skills. Our Community nurse works with you, your family, and other health professionals and caregivers to provide coordinated health care that addresses your physical, emotional, and social needs. Our goal is to help Hanover residents live safely and as independently as possible at home and in their community. There is no fee for these services, which include: Coordinating your care with your primary physician/agencies/resources; Providing health education, resource information and referrals; Serving as your advocate for your medical needs; Making home visits to assess your current health care needs; Assisting in scheduling medical appointments and helping ensure you can attend and effectively participate in visits with health care providers; Assisting in helping you obtain medical equipment; Assisting with questions about insurance, advance directives and long-term care planning.

The Aging Resource Center provides older adults, families, and community members with information, education, and support to help them live healthier and more informed lives.

Home Health Care includes skilled nursing care and physical, occupational and speech therapies. It helps patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home. Personal Care Services assists with daily tasks, such as bathing, grooming, light housekeeping and meal preparation, so people can continue to live in their homes independently.

Our Portsmouth Health Care Center offers primary care to keep our New Hampshire communities healthy. This facility conveniently offers on-site pharmacy, lab, and radiology services, as well as physical therapy and health education courses.

Various free support groups are offered including: caregiver, general health, disease specific, wellness, etc.

This agency offers a variety of health screenings, programs and support groups like: falls prevention, disease self management, blood pressure screenings, caregiver support groups, and more.

MSAA’s free services help improve lives. Programs include a Helpline with trained specialists; award-winning educational videos and publications, including MSAA’s magazine, The Motivator; safety and mobility equipment distribution; cooling accessories for heat-sensitive individuals; educational programs held across the country; MRI funding; and Lending Library among other services.

The Alzheimer's Association offers programs that cover a wide array of topics in small workshop sessions, trainings and in-depth multi-part series for caregivers, health care professionals, and people at all stages of Alzheimer's disease. Programs provided to persons with memory impairment, their families or caregivers, and the general public. Programs include: Know the Ten Signs; Effective Communication; Managing Dementia-Related Behaviors; Dementia Conversations; Coping With Early Memory Loss; Healthy Living for Your Brain and Body.

Trained leaders teach and lead attendees in exercises that help build bone density and strength using weight bearing and non-weight bearing exercises. The weights are provided at each session. This low impact program also helps with balance, flexibility, muscle strength, overall health and social relationships. The program helps to rebuild bone density and also helps with balance.
