Diabetes Care services available include: individual and group education and training, insulin pump and continuous glucose monitoring programs, the diabetes fitness program, and a diabetes support group.

Lamprey Health Care is a non-profit health care clinic. The clinic provides a wide range of primary health care services to people of all ages. Other services provided are counseling, HIV testing, chronic disease management, physicals, wellness programs, diabetes screening and education, and more. Financial assistance available, based on eligibility guidelines. Call and ask for a Patient Services Representative for more information.

A Peer Support Agency providing a non-medical approach to mental illness for adults eighteen (18) years and older. It offers: social/recreational activities, support/wellness groups, education, workshops, and more.

Individuals with disabilities can access their communities and increase or maintain their independence by finding assistance through GSIL's Access Modification Program. We can assist you with access issue evaluations, the construction of exterior ramps that meet all American Disabilities Association (ADA) and Barrier Free Design Code (BFDC) regulations, and the purchase and installation of accessibility equipment such as lifts, durable medical equipment, vehicle modifications and communication technology. We can assist you with home accessibility evaluations and solutions including, but not limited to: •Wheelchair Ramps •Vehicle Modifications •Grab Bars •Bathroom Modifications •Stair Lifts •Durable Medical Equipment

The UNH Cooperative Extension Nutrition Connections Program provides nutrition education and food resource management education to low-income adults and youth throughout the state. They use a variety of nutrition curricula based on the unique needs of clients. They work with clients on a one-to-one basis, small group basis and also via social marketing. The Cooking Basics program provides 6-8 free in-home sessions that teaches participants to hands on cooking and to create a recipie file of family favorites, demonstrates healthy choices, how to stretch the food dollars, staying active and more.

The purpose of the coalition is to increase awareness and collaboration among service providers for families with young children. We also support parents enrolling in childcare scholorship.

GSIL’s Information and Referral offers a wide range of topics on living more independently, such as follows: Discovering more about GSIL services and how to become a GSIL consumer, obtaining funding for adaptive equipment, home modification and adapted vehicles, locating assistance for finding and maintaining employment, learning about transportation options in the community, discovering how to obtain a service dog, learning about vendors, organizations and agencies that can provide goods and services, finding out about housing alternatives and funding assistance for housing.

The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. Services can be provided in a variety of settings such as your own home, child’s day care, neighborhood playground or other community setting.

The CDC provides information on a variety of topics including diseases and conditions; healthy living; workplace safety; environmental health; injury; violence and safety; global health; travelers health and more. They also provide locations to obtain a Covid-19 vaccine.

The Teen Clinic provides reproductive health services with the goal of reducing unintended pregnancy and the transmission of STD/HIV. The Teen Clinic provides comprehensive, confidential services in a teen friendly environment.

An array of skilled nursing and therapy for all ages to help patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home

Reproductive and sexual health care services includes but not limited to birth control, pregnancy testing, STD testing

Concentra offers occupational and urgent medical care, as well as physical therapy and wellness services. We treat everything from sprains and broken bones to coughs, colds, and flu.

Center offers seminars and classes for the general public. Some classes are free, some are offered at discounted rate.

A full-service primary care medical practice for people of all ages, including: • Primary care for adults • Pediatric care & child-development screenings • Prenatal care • Mobile health care for people experiencing homelessness and others with low incomes • Breast and cervical cancer screenings • Education and support for managing chronic diseases.

The registered dietician provides nutrition counseling in an effort to teach people about the impact of proper nutrition, and diets specific to health care needs. The challenge of achieving health and well-being is to find a balance that fits with your lifestyle. Each day, older adults gather at the center to meet new and old friends, enjoy entertaining programs, participate in exercise and craft classes, partake in lively discussion groups on social and political issues and use their skills to help others as volunteers. In addition to these important social programs, the center also provides older adults with nutritional lunches, health education and screenings and transportation to shopping and medical appointments.

Our Portsmouth Health Care Center offers primary care to keep our New Hampshire communities healthy. This facility conveniently offers on-site pharmacy, lab, and radiology services, as well as physical therapy and health education courses.
