Diabetes Self-Management Program is a comprehensive outpatient referral-based education program that empowers participants to take control and successfully manage their diabetes. Participation provides you with the opportunity to fully understand the rationale for early aggressive intervention to prevent complications from diabetes. Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

The Health Care for the Homeless program provides primary medical care, nursing case management, addiction counseling, eye care, mental health care, dental care, and health education to homeless people in Manchester, regardless of their ability to pay. Nurse case managers guide patients through the health care system, helping them to also to obtain social services and housing. HCH has a mobile medical van that goes to Manchester shelters and parks. Services for Deaf and Hard of Hearing individuals, families, and community. Services includes: Information Referral; Service Coordination / Advocacy Case Management; Support Group Information and social group; Support for Deaf and Hard of Hearing Students NH Deaf and Hard of Hearing Education Initiative Project; Family Sign Language Program; Interpreter Referral / Cart Referral and Emergency Medical Interpreting Services (EMIS); Deaf Blind Communication Equipment Program; Equipment and Material Loans Assistive listening devices, TTY, and resource materials for loan; Outreach Services/Community Education

Acute/sick visits and preventive/well-child visits are offered. During Well Child visits, children receive regular checkups, health screenings and immunizations based on the schedule recommended by the American Academy of Pediatrics.

On-site services include: primary and preventive care, nutrition counseling, internal medicine, pediatrics, outpatient mental health counseling, minor skin surgeries, walk-in services, care coordination and planned diabetic visits. Lab and x-ray services, and referrals to medical specialists, including dental care, can be coordinated to provide comprehensive care that meets your personal needs.

We partner with families to meet the developmental needs of the child for children birth to age 3. For those children found eligible, we offer services that include parent coaching, physical, occupational, speech and developmental therapies.

The WIC program provides supplemental foods, health care referrals, and nutrition education at no cost to the families of low-income pregnant, breastfeeding and non-breastfeeding post-partum women, and to infants and children up to 5 years of age.

Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

They use a variety of nutrition curricula based on the unique needs of clients. They work with clients on a one-to-one basis, small group basis and also via social marketing. The Cooking Basics program provides 6-8 free in-home sessions that teaches participants to hands on cooking and to create a recipie file of family favorites, demonstrates healthy choices, how to stretch the food dollars, staying active and more.
