The Ways2Wellness CONNECT Program links Community Health Workers (CHW)s with patients who are struggling with unmanaged chronic disease and experiencing barriers to achieving overall health and wellness to enhance quality of life. The program currently works with patients ages 55 and over with unmanaged chronic disease, such as hypertension, diabetes, congestive heart failure, COPD, etc. at no cost to the patient or provider.

REAP is a prevention program that is available to all older adults in the community. It is designed to help you take control of your life, to live a happy, healthy and independent lifestyle. They offer up to 6 free, confidential counseling and educational services to help you better understand and deal with the many changes you encounter.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

The Health Care for the Homeless program provides primary medical care, nursing case management, addiction counseling, eye care, mental health care, dental care, and health education to homeless people in Manchester, regardless of their ability to pay. Nurse case managers guide patients through the health care system, helping them to also to obtain social services and housing. HCH has a mobile medical van that goes to Manchester shelters and parks. Community Mental Health Center provides services to assist people with managing the symptoms and impairments resulting from mental illnesses. These include case management, therapy, functional supports, nursing and psychiatric services. The program also utilizes a number of nationally recognized evidenced based practices, including Illness Management and Recovery and Supported Employment.

Peer Recovery Support Specialists promote recovery, remove barriers, build recovery capital, connect recoverees with recovery support services; and encourage hope, optimism and healthy living. Peer Recovery Support Specialists often will assist individuals with removing barriers such as assisting with employment, housing, educational needs, connections to mental health resources when desired, connections to meetings through multiple pathways as well as recovery oriented social network events, activities and other wellness related opportunities. Peer Recovery Support Specialists also provide crisis response and resources for community members of all ages who may be struggling with substance misuse and are seeking treatment and recovery options.
