Wentworth-Douglass Hospital Care-Van is a service offered to eligible patients coming to Wentworth-Douglass Hospital for an extended course of treatment, most commonly Cancer treatments, Physical Therapy and Wound Healing treatments. Patients are picked up at residence, taken to hospital for treatment, then returned to their home. A family/friend or caregiver may accompany patient.

CVTC provides rides through volunteer drivers to non-emergency medical & social service appointments, grocery shopping, and rides to the bank, pharmacy and post office. Transportation is provided for anyone without access to transportation because of their age, ability, income, and/or a life circumstance.

In addition to transportation provided by bus, volunteer drivers also are available to help older adults.Some senior centers are able to offer long-distance medical transportation to those who need to reach a specialist or a major medical center not located in their local community. Concord Senior Transit (CST) is an origin-to-destination transportation for seniors 60 years and older in the Greater Concord Area. The vehicles are wheelchair accessible. Seniors riding the CST can easily access downtown Concord, go shopping, and get to medical appointments and recreational activities. Same-day rides are based on availability only.

Granite State Independent Living transportation reimbursement program (TRP) exists to support people with disabilities to who financially qualify to access their community. TRP can be used social outings such as movies, shopping, family gatherings, events, etc. This service is for consumers who do not have access to public transportation, or where public transportation doesn't meet their need. Reimbursement can be provided for a private car, or ride share programs such as Lyft, Uber, taxis and cost of bus fare or passes.

The CAPSC bus provides door-to-door, demand response transportation to anyone age 60 and over The suggested fare donation is $2 per roundtrip. Rides are provided to grocery stores, shopping malls, and discount shops. The bus is wheelchair accessible.

Concord Area Transit provides fixed route transportation Monday-Friday from 6:00am-6:30pm in the City of Concord, New Hampshire. CAT's three routes are the Crosstown Route, Heights Route, and the Penacook Route.

If you call CommuteSmart, 1-87-RIDEMATCH, we can arrange a ride home for you. If you would like to be reimbursed, you can call a taxi, rent a car, utilize Uber/Lyft or have a friend drive you home. CommuteSmart members are eligible for up to 5 rides per year.

CAREPLUS Ambulance Service, Inc provides a wide range of Emergency, Urgent Care and Non-Emergent Ambulance transportation services. Our services include ALS, BLS, Specialty Care Transportation, Bariatric Services and Neo-Natal Pediatric Specialty transport. Inter-facility transport services to 911 emergency calls are provided in a professional and timely manner by CAREPLUS. CAREPLUS Ambulance Service, Inc provides Chair Car Services throughout New Hampshire for area healthcare facilities and residents. Our fleet of vans are equipped with special tie-downs for oversized wheelchairs and electric wheelchairs.

RSVP finds rewarding volunteer opportunities for people age 55+ (primarily). Many, varied opportunities are available, and we will start new projects to suit your needs. Services by volunteers most frequently focus on health and wellness (including osteoporosis prevention, health information distribution, healthcare and senior living agencies, food security and independent living). We also place volunteers in schools to increase literacy for children and adults, and many other areas of community need. RSVP Caregivers provides transportation for medical or basic needs for frail elderly or disabled persons. Volunteers may also choose to provide visitation and companionship, telephone reassurance or assistance with small tasks.

Three types of transportation are provided. 1. Fixed route. 2. Deviation Route Service- Buses deviate off the established route pattern up to one quarter mile to pick up and drop off passengers. 3. Demand response- Provided by the Volunteer Driver Program for senior citizens and individuals with disabilities.

Transportation to medical appointment accompanied by a registered nurse who may act as liaison with doctor for clients who may be on oxygen, have dementia, etc.

Rides are provided for medical and personal appointments: doctor/dentist appointments, lab tests, pharmacy pick-ups, grocery and other shopping, errands, hair appointments, etc. We will accommodate extended-area trips (i.e., Boston, Manchester, Lebanon, Portland, etc.) for medical appointments only. **Rides are provided by volunteers in their personal vehicles. We are unable to accommodate wheelchairs.
