At Villa Crest Nursing and Retirement Center our aim is to provide the level of service that meets the residents needs. Whether a short stay for rehabilitation therapy is appropriate or a new place to call home, Villa Crest has the appropriate services. Opening New Doors Senior Living Placement Services educates clients about the costs, medical care, amenities and accommodations of individual Assisted Living and Memory Care Communities. We focus our client's search on communities who have the highest likely-hood of meeting their needs, schedule and accompany our clients on tours and continue to provide support throughout move-in.

New Hope New Horizons (NHNH) provides life enriching services to adults of all abilities so that they may achieve their personal hopes and dreams. These services include employment, day and community outreach, and residential support in Cheshire County

Not everyone can live alone, and certainly not everyone can afford 24/7 supportive care. Harbor Homes and Harbor Care have several housing programs to keep people in their communities, healthy and connected, with the 24/7 support of staff. We have several programs in Nashua for people who cannot live alone due to significant behavioral health disorders or other disabilities. These programs may be a permanent home or a stepping stone on the way to more independent community living. Programs include: --Group Homes are large licensed, shared homes for clients with significant mental illness. Clients have their own bedrooms, and share kitchens, living areas, baths and other common space. Group homes are staffed 24/7 to assist clients. --Glencliff to Community Program: is a group home for elderly individuals with significant physical and mental illnesses who were previously residing in the state-run nursing home, Glencliff. --Safe Haven is a shared home for up to five individuals with significant disabilities who are experiencing homelessness, but are not yet connected with community services. Individuals work with staff to manage the symptoms of their illnesses before moving onto more independent permanent housing. --NH Hospital Transitional Housing Program reintegrates individuals with significant behavioral health disorders who have been living in NH Hospital back into the community.

Three levels of care are available. 1. Skilled inpatient nursing home care with rehabilitation services like; physical therapy, occupational therapy, and speech therapy. Stays are short term with the goal of returning home. 2. Intermediate level, this is permanent long term nursing home care. 3. Ernest P. Barka Assisted Living Facility is for seniors who can no longer live unassisted. Staff is available 24/7.

We advocate and oversee supports and services to ensure continuity of care. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

Permanent supportive housing for adults with mental illness and very low income. Fellowship Housing offers a continuum of housing choices and support based upon the individual's needs. The housing offers a foundation upon which the individual can achieve and maintain a level of recovery from psychiatric symptoms in order to live in the community as a citizen, neighbor, friend, worker, etc. Fellowship Housing supplies heat, water and electricity and maintains the buildings. The renter is responsible for obtaining their own food and household supplies, and also pays for telephone and cable TV.

We work collaboratively with families and agencies, building expansive support networks around participants, which include crisis management, behavior support, and goal achievement. Individualized Service Options (ISO) is a life-sharing home, where participants live with a home provider in their home (or vice versa!).

Too many people never really escape homelessness. Often struggling with chronic illness, they run into crisis after crisis—lose a job, get evicted, lose healthcare—and get short-term help before the cycle of homelessness repeats. Utilizing a “Housing First” model of care, combined with long-term housing and supports, we stop the cycle of chronic homelessness and get people on track to a stable life. How It Works: Harbor Care often prioritizes its housing for veterans and individuals and families living with disabilities, including behavioral health disorders such as mental illness or substance abuse disorders, physical disabilities, or those living with chronic illness such as HIV/ AIDS. Our housing comes in a variety of forms – from shared houses to apartment complexes to “vouchers” that allow a client to choose their own home within the community. In all cases, rent remains affordable, with clients paying a portion of their income towards rent. Our housing often comes bundled together with services such as case management, to help clients find employment, obtain entitlements and benefits for them and their families, and avoid returning to homelessness. Programs include: --Permanent Supportive Housing Program, for chronically homeless individuals through federal subsidies (non-voucher). --Mainstream Voucher Program, for chronically homeless individuals through a federal voucher. --Boulder Point, a project-based HUD-VASH (Veterans Affairs Supportive Housing) voucher program for veterans. Harbor Homes and Harbor Care also operate other federally-subsidized properties, which helps us offer lower rents to qualified community members.
