This a volunteer run transportation service for eligible residents who are age 55 or older or who have a disability/medical challenge that prevents them from driving. Passengers must be able to get to and from a vehicle with little or no assistance. *****Walkers, rollators, and other equipment that can be stowed in an average car are fine. **Wheelchair transportation in our accessible minivan is available if a properly trained volunteer driver is available. Once registered, to arrange a ride call at least a week before the ride is needed. Because all drivers are volunteers, we can never guarantee a ride.

Harbor Care often prioritizes its housing for veterans and individuals and families living with disabilities, including behavioral health disorders such as mental illness or substance abuse disorders, physical disabilities, or those living with chronic illness such as HIV/ AIDS. Our housing comes in a variety of forms - from shared houses to apartment complexes to "vouchers" that allow a client to choose their own home within the community. In all cases, rent remains affordable, with clients paying a portion of their income towards rent. Our housing often comes bundled together with services such as case management, to help clients find employment, obtain entitlements and benefits for them and their families, and avoid returning to homelessness. Programs include: --Permanent Supportive Housing Program, for chronically homeless individuals through federal subsidies (non-voucher). --Mainstream Voucher Program, for chronically homeless individuals through a federal voucher. --Boulder Point, a project-based HUD-VASH (Veterans Affairs Supportive Housing) voucher program for veterans. --Harbor Homes and Harbor Care also operate other federally-subsidized properties, which helps us offer lower rents to qualified community members. Gateways adult day has daily activities for participants. Examples of activities are: music, exercise, arts and crafts, discussions, and more. The program is staffed by nurses,aids,and activity personnel. Breakfast, lunch and mid-afternoon snacks are provided. Referrals to private transportation companies provided.

Students in the dental hygiene program offer cleanings, x-rays, and sealants at a reduced rate. Appointment length ranges from 1 1/2 hours to 3 hours depending on the procedure being performed, and requires more than one visit for completion of services. WE ARE NOT ABLE TO PERFORM ANY RESTORATIVE DENTAL WORK OR EXTRACTIONS.

Ibis provides daily engagement, real-time monitoring, and personalized care coordination from the members home. It combines technology, proactive communication, and human connection to support whole-person health. Services include: Ibis tablet to report daily vital signs, medication intake and overall well-being Case Management with a Member Advocate who help coordinate care, explain benefits, and connect to services Nurse and physician connection to oversee support Daily monitoring of health Whole-person care planning: Addressing medical, social, emotional and functional challenges Preventive and Proactive: Detects issues early, often results in shorter hospital stays, fewer ER visits and better chronic condition management.
