The Moore Center serves people of all ages with Intellectual & Developmental Disabilities and Acquired Brain Disorders. - We offer Family-Centered Early Supports and Services for children from birth - 3 years. *Adult Services for Age 21 years old and older includes: Residential Services, Community Based Services, and Employment Services. *Children Services for Ages 3 to 13 includes: family-directed, supports and services that assist families such as respite, consultation, information and referral and "In Home Supports," a specific waiver service. *Children Services for Ages 14 to 21: Services includes: Family Support Services, transition from the school system to adult services. *our Kinship program pays family caregivers for providing care for their loved ones in their home

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

All living situations are designed to suit individual needs, whether through staffed residential homes, Enhanced Family Care, or companion and roommate programs. Caring, well-trained staff provide hands-on, reliable program supervision and case management services. Individuals also have the assurance that all safety and health care needs will be met through 24-hour on-call services, including nursing and administration support, as well as overnight assistance and supervision if needed. Certified professionals, including supervision of medication administration, physician appointments, and support with hospitalizations, also provide comprehensive Health Care Management. Disaster preparation and plans are also in place to address emergency situations.

Day activities include: caring for the farm animals and the gardens, working in the greenhouses, housekeeping, cooking/baking, clerical/office support, working in the farm stand or petting farm, arts/crafts, nature walks, and horseback riding. Transportation available within a 30 min. radius

Neuro-Resource Facilitation (NRF) assist individuals and their family members to navigate the New Hampshire service system.

Farmsteads of New England provides supportive services in a farming environment to adults who have autism and other developmental disabilities. People who live within commuting distance of the farmsteads are welcome for structured day services.
