REAP is a prevention program that is available to all older adults in the community. It is designed to help you take control of your life, to live a happy, healthy and independent lifestyle. They offer up to 6 free, confidential counseling and educational services to help you better understand and deal with the many changes you encounter.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

The Health Care for the Homeless program provides primary medical care, nursing case management, addiction counseling, eye care, mental health care, dental care, and health education to homeless people in Manchester, regardless of their ability to pay. Nurse case managers guide patients through the health care system, helping them to also to obtain social services and housing. HCH has a mobile medical van that goes to Manchester shelters and parks. Mental health services for ages 3 and older. Services include, but not limited to: individual, couples, group, family therapy and Psychological and Neuropsychological assessments and testing. Support services for established clients may include: case management, vocational, community, and home based services.

Community Mental Health Center provides services to assist people with managing the symptoms and impairments resulting from mental illnesses. These include case management, therapy, functional supports, nursing and psychiatric services. The program also utilizes a number of nationally recognized evidenced based practices, including Illness Management and Recovery and Supported Employment.

Lakes Region Mental Health Center provides a wide range of behavioral health services to children, adolescents, young adults and their families. In some cases, it is important to include families in the treatment plan whenever possible. Services includes: Individual, Group Family Therapy; Case Management; Functional Support Services; Acute Care Services; Mobile Crisis Response Team; Child to Adult Transition; Trauma Focused Cognitive Behavioral Therapy; Dialectical Behavioral Therapy; Art Therapy

Riverbend’s Children’s Services clinicians provides comprehensive, evidence-based treatment to help young people and their families identify and resolve issues that keep them from success.
