In an effort to continue providing critical care to our clients, we have modified our service delivery on a program-by-program basis. FAMILY WELLNESS HOME VISITING | COMMUNITY ACTION PARTNERSHIP OF STRAFFORD COUNTY
The Family Wellness Home Visiting program helps at-risk-families, focusing on prevention by offering emotional support and practical solutions to managing family life as well as in home assistance with parenting skills and household management and promoting self-sufficiency by coaching families to be able to access community services and advocate for themselves. Helping to access employment programs, addiction and recovery programs, attend medical/social service appointments, school meetings and recreational activities.

Our staff helps NH and VT families find resources and services to meet basic needs, make connections, and get support. We offer a wide range of educational classes and groups for free, and all are welcome. For NH families, we provide Healthcare Coordination for families raising children with chronic healthcare conditions, and Community-Based Voluntary Services to preserve families at risk. We also offer diapers, and other products to meet basic needs.
