Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers home health aides, spiritual care counselors and bereavement counselors.

1. Skilled in home nursing services offered include: physical, occupational, speech therapy; IV drugs, shots, tube feedings, dressing changes, and more. 2. Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. Hospice (nursing care, pain management, patient and family support, etc.
Veterans who have served on active duty in the armed forces of the United States as defined by 38 U.S.C. section 101,or in reserve units or in the New Hampshire army or air national guard and have been honorably discharged there from are eligible for admission. The veteran shall have been a resident of the state of New Hampshire for at least one year next preceding the veteran's application. II. The one-year residency requirement shall be waived for the following veterans: (a) A veteran whose DD214 certificate or any document used by the veterans' home for admission states a New Hampshire address as his or her home of record upon discharge from active duty. (b) An active duty veteran who has a New Hampshire address as his or her home of record and who is being discharged directly from a medical facility. III. The care of veterans under this section includes nursing care, dementia, psychiatric services, and other related services and programs for any veteran otherwise eligible for admittance. Such services and programs shall be provided at the veterans' home or contracted through other organizations and facilities, both within and outside of New Hampshire.

Palliative Care is specialized medical care for people living with unmanaged symptoms of a serious illness including Heart Failure, COPD, Cancer, and other chronic illnesses. Patients can receive Palliative Care services while still receiving curative treatment for their disease, and it is appropriate at any age and at any stage of a disease.
Palliative care can help patients of all ages during and after curative treatment at any stage of their illness. The goal is to improve quality of life while considering a patient’s unique needs. Our palliative care core team includes a palliative care physician and/or nurse practitioner, nurse, social worker, and spiritual counselor. They partner with you, your loved ones, and your doctor from your primary treatment team to address your physical, emotional, social, and spiritual needs to improve symptoms such as: - Pain - Shortness of breath - Fatigue - Constipation - Nausea - Loss of appetite - Difficulty sleeping - Depression - Stress

The Hospice House is a home that includes 24-hour skilled nursing care for terminally ill patients and their families. Some of the amenities of the Hospice House include a pull-out sofa bed in each room for family and friends to sleep, a personalized menu, a family kitchen area, and a Multi-Faith Meditation Room. There is also access to a computer for e-mail and Internet, and an outside walking path and garden area.

Hospice services are offered to people with life-limiting illness who have chosen to discontinue curative treatment. With a focus is on ‘caring’, not ‘curing,’ people receive services in their home, a nursing home, a hospital or a residential facility. Many of the same services found in a hospital setting can be provided in a patient’s familiar surroundings. Care teams consist of doctors, nurses, social workers, chaplains, and specially-trained volunteer/bereavement coordinators who provide support and assistance at the request of the patient and the family.
