Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers home health aides, spiritual care counselors and bereavement counselors.

Home health, hospice and palliative care provided through North Country Home Health and Hospice agency. Home health care is available to anyone in need of everyday assistance after an acute illness, injury or exacerbation of a chronic illness. Hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible.

The Elliot VNA offers a continuum of service and support to serve individuals of all ages. We can help with, hospice care, palliative care, recuperating from recent illness, recovering from a surgery, managing a chronic illness and more form the comfort of your home.

Full service hospital, open 24/7, providing emergency, urgent, medical, surgical, radiological, rehabilitation, laboratory, and other medical services.

1. Skilled in home nursing services offered include: physical, occupational, speech therapy; IV drugs, shots, tube feedings, dressing changes, and more. 2. Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. Hospice (nursing care, pain management, patient and family support, etc.

Veterans who have served on active duty in the armed forces of the United States as defined by 38 U.S.C. section 101,or in reserve units or in the New Hampshire army or air national guard and have been honorably discharged there from are eligible for admission. The veteran shall have been a resident of the state of New Hampshire for at least one year next preceding the veteran's application. II. The one-year residency requirement shall be waived for the following veterans: (a) A veteran whose DD214 certificate or any document used by the veterans' home for admission states a New Hampshire address as his or her home of record upon discharge from active duty. (b) An active duty veteran who has a New Hampshire address as his or her home of record and who is being discharged directly from a medical facility. III. The care of veterans under this section includes nursing care, dementia, psychiatric services, and other related services and programs for any veteran otherwise eligible for admittance. Such services and programs shall be provided at the veterans' home or contracted through other organizations and facilities, both within and outside of New Hampshire.

Exeter Hospital offers Palliative care. This hospital has a partnership with Massachusetts General Hospital Cancer Center to provide specialized inpatient and outpatient cancer care. Services include: radiation, surgery, genetic counseling and testing, and more. A nurse navigator helps patients through the treatment and recovery process.

Palliative Care concentrates on the conversations and navigation of advanced illness with an emphasis on personal goals and symptom management. Hospice Care focuses on physical, emotional and spiritual comfort, so individuals with a life limiting illness can live their last phase of life to the fullest wherever they call home.

Patients can receive Palliative Care services while still receiving curative treatment for their disease, and it is appropriate at any age and at any stage of a disease.

Palliative care can help patients of all ages during and after curative treatment at any stage of their illness. The goal is to improve quality of life while considering a patient’s unique needs. Our palliative care core team includes a palliative care physician and/or nurse practitioner, nurse, social worker, and spiritual counselor. They partner with you, your loved ones, and your doctor from your primary treatment team to address your physical, emotional, social, and spiritual needs to improve symptoms such as: - Pain - Shortness of breath - Fatigue - Constipation - Nausea - Loss of appetite - Difficulty sleeping - Depression - Stress

Outpatient Specialty Care Services includes: - Weight Management: work with our team of obesity medicine physicians, dietitians, and health coaches to get the guidance you need for successful weight management. - Pain Management services includes: evaluation and diagnosis to medical therapies, including both medications and special procedures to treat short-term, long-term or chronic pain. - Palliative Care addresses both the physical symptoms and the stress and worry that accompany a serious illness. Those living with illnesses such as cancer, heart disease, lung disease, kidney failure, dementia, among others, experience physical symptoms and emotional distress related to their diseases.

Palliative Care is a stand alone service providing Nurse Practitioner level of care in your home or place of residence. Palliative care is appropriate for people dealing with a serious, chronic illness. Our practitioners will collaborate with your physician and/or specialist to provide pain and symptom management and/or Goals of Care conversations. This is a consultative service.

The Hospice House is a home that includes 24-hour skilled nursing care for terminally ill patients and their families. Some of the amenities of the Hospice House include a pull-out sofa bed in each room for family and friends to sleep, a personalized menu, a family kitchen area, and a Multi-Faith Meditation Room. There is also access to a computer for e-mail and Internet, and an outside walking path and garden area.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

Hospice services are offered to people with life-limiting illness who have chosen to discontinue curative treatment. With a focus is on ‘caring’, not ‘curing,’ people receive services in their home, a nursing home, a hospital or a residential facility. Many of the same services found in a hospital setting can be provided in a patient’s familiar surroundings. Care teams consist of doctors, nurses, social workers, chaplains, and specially-trained volunteer/bereavement coordinators who provide support and assistance at the request of the patient and the family.
