Provides individualized care and services to support terminally ill clients, their families, and caregivers in the clients' homes. Palliative Care concentrates on the conversations and navigation of advanced illness with an emphasis on personal goals and symptom management. Hospice Care focuses on physical, emotional and spiritual comfort, so individuals with a life limiting illness can live their last phase of life to the fullest wherever they call home.

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

Emergency Services at Littleton Regional Healthcare is an integrated team providing urgent or emergent care to critically ill and/or seriously injured persons.

1. Home Health Aide Services are delivered by certified nursing assistants who assist with daily living activities and personal care. Home Health Aides help patients get in and out of bed, bathe, dress and walk. Aides can provide more specialized care such as exercise routines, blood draws, and simple dressings. 2. Nurses provide skilled assessment, planning ,monitoring, treatment and evaluation in the home setting. 3. Rehabilitation therapy offers physical therapy and occupational therapy.

Hospice services are offered to people with life-limiting illness who have chosen to discontinue curative treatment. With a focus is on ‘caring’, not ‘curing,’ people receive services in their home, a nursing home, a hospital or a residential facility. Many of the same services found in a hospital setting can be provided in a patient’s familiar surroundings. Care teams consist of doctors, nurses, social workers, chaplains, and specially-trained volunteer/bereavement coordinators who provide support and assistance at the request of the patient and the family.

For those who choose the comfort and dignity of their own surroundings at the end of life, Hospice at HCS provides comprehensive care and support to those with a diagnosis of a terminal illness. Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers home health aides, spiritual care counselors and bereavement counselors.

Hospice offers comfort care, symptom management, spiritual and emotional support, information on caring for loved ones, admissions and on-call support 24 hours a day, bereavement services, and volunteer support. We provide services for people of all ages at home, in assisted living facilities, retirement communities, a skilled facilities, and at Granite VNA Hospice House.

This is an inpatient hospice unit. It has private rooms and bathrooms. Hospice trained registered nurses and hospice aides provide onsite care 24/7. Social workers and spiritual care coordinators care for the emotional and spiritual needs of both the patient and family. Ongoing grief counseling and bereavement support is provided for at least 13 months following the death of a loved one.

Full Service Hospital

Veterans who have served on active duty in the armed forces of the United States as defined by 38 U.S.C. section 101,or in reserve units or in the New Hampshire army or air national guard and have been honorably discharged there from are eligible for admission. The veteran shall have been a resident of the state of New Hampshire for at least one year next preceding the veteran's application. II. The one-year residency requirement shall be waived for the following veterans: (a) A veteran whose DD214 certificate or any document used by the veterans' home for admission states a New Hampshire address as his or her home of record upon discharge from active duty. (b) An active duty veteran who has a New Hampshire address as his or her home of record and who is being discharged directly from a medical facility. III. The care of veterans under this section includes nursing care, dementia, psychiatric services, and other related services and programs for any veteran otherwise eligible for admittance. Such services and programs shall be provided at the veterans' home or contracted through other organizations and facilities, both within and outside of New Hampshire.

Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. Home Health Aide and Homemaker Services. Part-time care for convalescents or handicapped people may include meal preparation, light housekeeping, shopping, personal, and health care. Hospice Program. In addition to 24-hour availability of a visiting nurse, this program provides trained volunteers to counsel terminally-ill patients and their families during the illness and in the bereavement period. Children's Health Care: Child Health Clinics in all areas provide complete health care for low-income children. Immunization and preventative health education for preschool children are provided in home and clinic settings.

The Hospice House is a home that includes 24-hour skilled nursing care for terminally ill patients and their families. Some of the amenities of the Hospice House include a pull-out sofa bed in each room for family and friends to sleep, a personalized menu, a family kitchen area, and a Multi-Faith Meditation Room. There is also access to a computer for e-mail and Internet, and an outside walking path and garden area.
