For those who choose the comfort and dignity of their own surroundings at the end of life, Hospice at HCS provides comprehensive care and support to those with a diagnosis of a terminal illness. Care can be provided at home, in assisted living or in long term care facilities. Care is provided by a team of nurses, medical social workers home health aides, spiritual care counselors and bereavement counselors.

Our Home Care Program provides high-quality, cost effective alternatives to institutional care. A team of nurses, therapists, social workers, aides and dietitians work under the order of a physician to provide patients and their families with essential healthcare services in the comfort of their home. In developing an appropriate plan of care for each patient, staff are trained to utilize the varied expertise of the home care team to provide the maximum care necessary for recovery. This team works together with the patient and family to promote independence.
The home care team assists in making those individuals who are recovering from surgery or a serious illness feel more comfortable and safe by providing comprehensive care in the home. A registered dietitian is available for nutritional counseling, and a registered nurse is on call 24 hours a day. Members of the home care team also include medical social workers who can help identify and manage psychosocial problems that arise from an individual's illness or medical treatment. The social worker also can provide counseling and assist with access to appropriate financial and community resources. Home health aides provide help to people who need assistance with daily living tasks. These certified nurses aides assist with bathing, dressing and food preparation, all under the supervision of a registered nurse.
Our experienced Maternal/Child Health staff works closely with families and their physicians to provide optimal pediatric, neonatal and maternal health care.
Services included in this program are: registered nurse visits to conduct skilled nursing assessments, education, direct care and coordination of services, including:
Prenatal/high risk visits
New mom/baby visits
Newborn intensive care follow-up visits
Pediatric/adolescent visits (0-18 years)
It is the hospice philosophy that the physical, emotional and spiritual needs of terminally ill patients and their caregivers can best be met by the services of an interdisciplinary team of trained professionals and volunteers. VNA Home Health and Hospice nurses provide skilled nursing care including physical assessment, patient and family teaching pain and symptom control monitoring and a registered nurse on call 24 hours a day.

Hospice services are offered to people with life-limiting illness who have chosen to discontinue curative treatment.
With a focus is on ‘caring’, not ‘curing,’ people receive services in their home, a nursing home, a hospital or a residential facility. Many of the same services found in a hospital setting can be provided in a patient’s familiar surroundings.
Care teams consist of doctors, nurses, social workers, chaplains, and specially-trained volunteer/bereavement coordinators who provide support and assistance at the request of the patient and the family.

Veterans who have served on active duty in the armed forces of the United States as defined by 38 U.S.C. section 101,or in reserve units or in the New Hampshire army or air national guard and have been honorably discharged there from are eligible for admission. The veteran shall have been a resident of the state of New Hampshire for at least one year next preceding the veteran's application.
II. The one-year residency requirement shall be waived for the following veterans: (a) A veteran whose DD214 certificate or any document used by the veterans' home for admission states a New Hampshire address as his or her home of record upon discharge from active duty. (b) An active duty veteran who has a New Hampshire address as his or her home of record and who is being discharged directly from a medical facility.
III. The care of veterans under this section includes nursing care, dementia, psychiatric services, and other related services and programs for any veteran otherwise eligible for admittance. Such services and programs shall be provided at the veterans' home or contracted through other organizations and facilities, both within and outside of New Hampshire.
