The Early Supports & Services Coordinator is responsible for coordinating the intake, family support and the services provided by each agency. The Early Support team may include speech and language pathologists, occupational and physical therapists, educators and early childhood development specialists. 1. Skilled in home nursing services offered include: physical, occupational, speech therapy; IV drugs, shots, tube feedings, dressing changes, and more. 2. Long term services offered include: help with daily living activities, personal care services (bathing, dressing and eating), homemaking (laundry, groceries, cleaning and other routine household tasks). 3. Hospice (nursing care, pain management, patient and family support, etc.

Breast and cervical cancer screenings. Program site of the Let No Woman Be Overlooked Breast and Cervical Cancer Program, a program of the NH Dept. of Health & Human Services that provides free breast and cervical cancer screenings to women who are between 18 & 64 years old, have no health insurance coverage for these tests, and meet income guidelines.

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

We partner with families to meet the developmental needs of the child for children birth to age 3. For those children found eligible, we offer services that include parent coaching, physical, occupational, speech and developmental therapies.

Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

Provides developmental therapeutic early intervention services to infants and toddlers (birth to age three) who have an established medical/genetic condition, 33% developmental delay and/or who are at-risk of developing a substantial developmental delay.

Early Intervention includes speech, occupational, and physical therapy; counseling; and education interventions for children up to age 3 with a confirmed or suspected developmental delay or disability.

These in-home services help people regain mobility and functionality to reach their highest possible level of independence.

Outpatient Rehabilitation services includes Physical, Occupational, Speech and Language Therapy. PT work closely with physicians, patients and their love d ones to develop an individualized treatment plan to improve their ability to move, manage pain and prevent further disability. OT works with patients to develop Daily Living Activities such as bathing, dressing, eating, preparing meals and housework. Speech therapists help improve language skills and ability to communicate.
