You maintain full control over your plan, know­ing that your IL Services Coordinator is there to answer questions and make recommendations when you need support. If you are eligible for Services Coordination, your IL Services Coordinator will: Talk with you one-on-one, either in person or on the phone Listen to your needs Listen to your short- and long-term goals Document your stated goals in an Independent Living Plan Refer you to people and services that can help you achieve your goals, both at Granite State Independent Living and other agencies Act as a sounding board as you work toward your goals Help you modify your plan when you meet goals and when you experience changes in your life Model independent living skills and attitudes Work with you as long as you want to keep your Independent Living Plan active
