Comprehensive Outpatient Community Mental Health Services for Adults and Children. Services may include: Case Management, Outpatient Therapy, Psychiatric Services, Functional Supports and Services, Supported Employment Services, Assertive Community Treatment (ACT), Illness Management and Recovery, Financial Case Management, Infant/Early Childhood Mental Health, Parenting Education

Family Centered Early Supports & Services (FCESS), also known as ESS, provide resources to children from birth to three years of age and their families, including a developmental evaluation, therapeutic interventions, guidance, instruction, referral to community resources and emotional support.

• Children Services: Our specialists work with children experiencing serious emotional difficulties such as depression, attention deficit and hyperactivity disorder, autism, disruptive behaviors, substance use disorders, and trauma associated with emotional, physical or sexual abuse.
• Adult Community Support provides a multi-faceted range of services with a goal of supporting restoration, rehabilitation and recovery for clients with severe and persistent mental illnesses such as schizophrenia, mood and depressive disorders.

Our inpatient rehabilitation hospital uses an interdisciplinary team approach that includes physical, speech and occupational therapists, rehabilitation physicians, rehabilitation nurses, case managers, dietitians and more, combined with our advanced technology and expertise, to help you achieve your goals. You will receive at least three hours of therapy five days per week while under the constant care of registered nurses, many of whom specialize in rehabilitation, and frequent independent private practice physician* visits.

St Vincent de Paul Rehabilitation and Nursing Center offers care for short-term patients and residents. St Vincent de Paul Rehabilitation and Nursing Center has three distinct nursing units designated for residents according to care needs including short-term rehabilitation and skilled nursing, dementia and memory impairment, and long-term care. Almost two thirds of St. Vincent's 80 licensed beds offer private accommodations. Additionally, religious services are regularly provided in the chapel. JCAHO Accredited, Medicare and Medicaid approved.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

Services for adults and children with developmental disabilities and/or acquired brain injuries include case management, day programs, employment programs, family support and residential services. Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. Emergency Services at Littleton Regional Healthcare is an integrated team providing urgent or emergent care to critically ill and/or seriously injured persons.

ACT's Travel Training program teaches individuals how to use public buses, read schedules, plan routes, and how to access transportation programs. Presentations are also given to service providers whose clients include seniors and individuals with disabilities.

Early Intervention includes speech, occupational, and physical therapy; counseling; and education interventions for children up to age 3 with a confirmed or suspected developmental delay or disability.

Rehabilitation Services of Concord Hospital provides comprehensive rehabilitation services to help patients regain their functional independence by allowing them to return to normal daily self-care, work and leisure activities.

Outpatient Rehabilitation services includes Physical, Occupational, Speech and Language Therapy. PT work closely with physicians, patients and their love d ones to develop an individualized treatment plan to improve their ability to move, manage pain and prevent further disability. OT works with patients to develop Daily Living Activities such as bathing, dressing, eating, preparing meals and housework. Speech therapists help improve language skills and ability to communicate.

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.

Services may include trainings, informational meetings, parent/caregiver or sibling support groups, and help to connect to other appropriate community services. Additional Services may include: Service Coordination, Respite, High School Transition Supports, In Home Support Services, Personal Care, Mobility and Physical Functioning, Consultation and Education, Functional Supports and Services.
