The rooms and common areas of this 51-bed center are all on one level allowing for convenient access to common dining, activity, and outdoor areas. Provides skilled nursing and long term nursing services, including a specialized Alzheimer's care. Hospice or "palliative" care is available if curative or restorative care is no longer appropriate and desired for terminally ill individuals. A private hospice room is furnished to accommodate extended visits by family members. JCAHO Accredited, Medicare and Medicaid approved.

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25.
