The rooms and common areas of this 51-bed center are all on one level allowing for convenient access to common dining, activity, and outdoor areas. Provides skilled nursing and long term nursing services, including a specialized Alzheimer's care. Hospice or "palliative" care is available if curative or restorative care is no longer appropriate and desired for terminally ill individuals. A private hospice room is furnished to accommodate extended visits by family members. JCAHO Accredited, Medicare and Medicaid approved.

Home Health Care includes skilled nursing care and physical, occupational and speech therapies. It helps patients recover from surgery, illness, injury or complications from a chronic condition in the comfort of home. Personal Care Services assists with daily tasks, such as bathing, grooming, light housekeeping and meal preparation, so people can continue to live in their homes independently.

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. Family Support Services to school age children are provided through the Family Support Department at Community Crossroads. Supporting families may include recreational opportunities, respite services, ongoing trainings, assistance in accessing environmental modifications or adaptive equipment, information and referral, periodic behavioral consultation & services, attendance at school meetings, assistance with planning for transition from early supports and services to school, and to adult services; and assistance with applying for state and federal benefits.

We partner with families to meet the developmental needs of the child for children birth to age 3. For those children found eligible, we offer services that include parent coaching, physical, occupational, speech and developmental therapies.

Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Community Mental Health Center provides services to assist people with managing the symptoms and impairments resulting from mental illnesses. These include case management, therapy, functional supports, nursing and psychiatric services. The program also utilizes a number of nationally recognized evidenced based practices, including Illness Management and Recovery and Supported Employment.

Rehabilitation Services of Concord Hospital provides comprehensive rehabilitation services to help patients regain their functional independence by allowing them to return to normal daily self-care, work and leisure activities.

Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Service Coordinators are responsible for the planning, implementation and coordination of services an individual receives. Service Coordinators also monitor the services an individual receives. Services may include: Family Support: Respite Care, High School Transition Supports and Financial Case Managers, Residential Services, Enhanced Family Care, Staffed Homes, Vocational Services, Community Based Day Services and Participated Directed & Managed Services

Program provides specially trained dogs to assist individuals with autism, deaf and hard of hearing and facility dogs for full time professionals such as physicians, teachers, counselors and licensed therapists in the treatment of and work with their clients.

PathWays provides supports for children with disabilities, adults with developmental disabilities or brain injuries in everyday life to maintain their independence and well-being is what we do here at Pathways. Child Find is a component of the Individuals with Disabilities in Education Act (IDEA) that requires states to identify, locate, and evaluate all children with disabilities, aged birth through 21 yrs of age, who are in need of early intervention or special education services.

Adult Service Coordination team at MDS assists adults with developmental and related disabilities as they identify personal goals, and then secures the supports and resources required to achieve them. Children's Service Coordination is a resource to families who have a child with a developmental disability or chronic health condition.
