Our inpatient rehabilitation hospital uses an interdisciplinary team approach that includes physical, speech and occupational therapists, rehabilitation physicians, rehabilitation nurses, case managers, dietitians and more, combined with our advanced technology and expertise, to help you achieve your goals. St Vincent de Paul Rehabilitation and Nursing Center has three distinct nursing units designated for residents according to care needs including short-term rehabilitation and skilled nursing, dementia and memory impairment, and long-term care. Almost two thirds of St. Vincent's 80 licensed beds offer private accommodations. Additionally, religious services are regularly provided in the chapel. Individuals served by NHS may receive service coordination to ensure their support needs are being addressed. Rehabilitation Services of Concord Hospital provides comprehensive rehabilitation services to help patients regain their functional independence by allowing them to return to normal daily self-care, work and leisure activities.

Outpatient Rehabilitation services includes Physical, Occupational, Speech and Language Therapy. PT work closely with physicians, patients and their love d ones to develop an individualized treatment plan to improve their ability to move, manage pain and prevent further disability. OT works with patients to develop Daily Living Activities such as bathing, dressing, eating, preparing meals and housework. Speech therapists help improve language skills and ability to communicate.

The Community Support Services (CSS) department provides a variety of tools and resources to assist people with disabilities to live and work in the community. The CSS department directs the Project Search work-based training program.
