The Mobile Health Service provides critical health-related services out into the community.

The Families First Medical Van will be at the St. Vincent de Paul in Exeter on the 2nd Monday of each month. Walk-in are welcome

Mobile health teams bring medical and social work services to places like shelters, soup kitchens and public housing. Services provided at clinics include preventive health screenings; sick visits; and chronic-disease management. The team will also link patients with staff at Families First and Goodwin Community Health who can help with eligibility determination and applications for housing assistance, food stamps, WIC, TANF, disability benefits, Medicare, Medicaid, other health insurance, veterans benefits and more. Clinic staff can also help patients access substance misuse and mental health counseling, family support, specialty care, medications, and labs and diagnostic testing. ** As of September 2024, clinics were offered in Dover, Rochester, Portsmouth and Exeter. Clinic locations are subject to change.

Nurse case managers guide patients through the health care system, helping them to also to obtain social services and housing. HCH has a mobile medical van that goes to Manchester shelters and parks. COVID testing and vaccines to home-based individuals. Paxlovid treatment COVID-19 available. May help treat individuals age 12 and up who have developed Covid-19 symptoms in the last 5 days, and are at increased risk of severe disease.

Free home visit by an EMT to help get your asthma under control. This service is available to all ages. Vists are tailored to your needs but can include 1) help with your medications 2) help identifying and managing triggers in the home 3) review of your symptoms and treatments 4) connections to community resources.

Mobile IV Therapy
